Please bring your personal identification documents with a photo, such as a driver's license, passport, my number card, Basic Resident Register card with photo, etc. for those who come to the counter.

※If the identity verification document does not have a photo such as a health insurance card, or if a certificate issued by an organization other than a public office, you may be asked to present other identification documents or ask questions .
※If you are a family member who does not fall under any of the person recorded on the family register tag, his spouse, or a direct relative, please state the reason for the claim in detail. If the reason for the request is not clear, we may ask you to provide materials.
※If there is no justifiable reason, a certificate cannot be issued. If proxy/agent is required, a power of attorney is required.
A fee of 300 yen per copy will be charged.
(If you get it at a convenience store, it will be 250 yen.)

If you are in Yokohama, both Address and permanent domicile, you can use your My Number Card to obtain a certificate with your own multi-copy machine installed at convenience stores nationwide.
When using the service, you will need the PIN (4 digits) of the digital certificate for user certification.
Usage time is from 9:00 to 17:00 on weekdays (excluding system suspension days).
A fee of 250 yen per copy will be charged.

If you have a My Number Card, a smartphone that supports My Number Card, or a credit card, you can apply for a certificate with you listed online.
To use it, you need the PIN (6 to 16 digits) of the digital certificate for signature.
In addition, 300 yen per copy + postage will be charged.
