Where should I report divorce registration form (divorce through consultation)? Also, what should I bring when submitting a report?

Notification and certification (family register, resident certificate, etc.)

When submitting the divorce by consent/agreement, what you need to bring with you and the notification address are as follows. For details on how to write divorce registration form, please refer to the separate section.

What you need to bring during the meeting divorce registration form

・divorce registration form 1 copy (notification form is common throughout the country.)
At the window of the ward office Family Registry Division Family Registry Section or the administrative service corner, you will be given the form of divorce registration form and instructions on how to write. (It is not handed over at the city hall.)
In addition, we can download from page of electronic application, report service of Yokohama-shi web site.
・To verify the identity of applicant, you will need to present identification documents with a photo (driver's license, passport, my number card, etc.).
If your identity verification document does not have a photo, such as a National Health Insurance Card, you may be asked to present another identification document or ask questions. (If you bring the written notice, you may be a messenger other than applicant.)
・If a person who is not the leader of family register refers to a married person after divorce, a separate notification is required. (Please notify us within 3 months from the date of divorce.)
・If the couple has an unmarried child, a separate notification is required when transferring the child to the new family register after divorce. (In divorce registration form alone, the child family register, will not change.)

Reporting destination

・It will be the "Ward Office Family Registry Division Family Registry Section" in the applicant (husband / wife) or the ward where permanent domicile is located (either one is acceptable). (It cannot be handled at the Administrative Service Corner and City Hall.)
・Even if you do not have Address or permanent domicile, you can still notify the ward office where your home address is located, such as when you are staying at your parents' home. (For details, please check with the "Ward Office Family Registry Division Family Registry Section" with the notification.)
・In the case of delivery to other than the ward office Hours, we will keep the documents at night and holiday reception desks, confirm the contents of the following Hours belt. If you cannot formally accept it, we will contact you, so be sure to indicate the Phone number that can be contacted during the daytime on weekdays.)
