Is it possible to make my own family register separate from the current family register?

If you want to create your own family register, separate from the current family register, you must submit a separate registration form to the ward office Family Registry Division Family Registry Section.

・The couple may not have their own domicile address. Only adults listed in family register, excluding the first and spouse, can submit a separate registration.
・Please note that once you register your registration, you will not be able to return to the original family register before your separation.
・family register, who has already been removed, cannot be separated.
・In addition, there is no change in the identity relationship such as parent-child relationship even if you submit a separate registration.

Please contact the ward office Family Registry Division Family Registry Section for details on notification of separation registration.

What you need to bring

1 copy of registration
 ・Notification forms are common throughout the country.
 ・You can hand out a form for registration at the ward office Family Registry Division Family Registry Section. We do not handle in administrative service corner and city hall.
 ・Those who can be applicant are adults other than the first and spouse of family register. If you are a minor, you will not be able to submit your registration.
 ・Those who bring the written notice may be a messenger other than applicant.

■Precautions
 From March 1, 2024, attachment of a family register certificate will no longer be required when submitting a family register notification. However, in the case of family register, which is not computerized, the attachment of family register cannot be omitted. Therefore, certificate of family register must be obtained at your permanent address and attach it to the notification form.

Reporting destination

It will be the "Ward Office Family Registry Division Family Registry Section" of the applicant (an adult other than the first person and spouse of family register) or the ward where permanent domicile is located (either one is acceptable), or the ward where the new permanent domicile is located.
(It cannot be handled at the Administrative Service Corner and City Hall.)

About the new permanent domicile after notification of separation

The new permanent domicile can be registered in any place in Japan.

How to fill in the Address column of the separation notification

・Please indicate the Address where you are currently registered.
・If you are going to change the Address, etc. at the same time as the separation notification and submit a notification to the new Address area, please fill out the form with the new Address and submit a notification regarding the change of Address.
(It is not possible to change the Address just by registering the separation.) Notification of changes in Address will be handled only by the ward office Hours. For more information, please check with "Ward Office Family Registry Division Registration Section". )
