Where can I get a single certificate (certificate to be submitted to a marriage information service / marriage consultant)?

Notification and certification (family register, resident certificate, etc.)

It can be taken at the municipal office of your permanent address. If you have permanent domicile in Yokohama City, you can take it at all ward offices Family Registry Division windows in the city (administrative service corner, city hall). 。 You can also request it by mail or online.

What to bring at the time of window request

To verify the identity of applicant, you will need to present identification documents with a photo (driver's license, passport, my number card, etc.).
If your identity verification document does not have a photo, such as a National Health Insurance Card, you may be asked to present another identification document or ask questions.
A proxy must also be submitted when proxy/agent is requested (including relatives listed in the same family register).

Reception hours (Family Registry Division Family Registry Section, each ward office)

Monday to Friday 8:45 a.m. to 5:00 p.m.
9:00 am to midnight on the 2nd and 4th Saturday of every month
(In Reception hours, lunch breaks are also available on Saturday (except for 2nd and 4th)
Excludes transfer holidays on Sunday, public holidays, national holidays, and New Year's holidays (December 29 to January 3 of the following year).

 ※Please use the form provided by the marriage information service / marriage consultant. If not available, you can also download it from the city website.
・Copy of identification documents (My Number Card, Driver's License, Health Insurance Card, etc.) with Address
 ※When you send a copy of your My Number Card, please send only the surface.
 ※When sending a copy of your health insurance card, please send the insurer number and the insured symbol / number so that they cannot be seen by black paint.
・Fixed amount of money, ordinary exchange (issued at the post office) or cash (registered cash) for commission (300 yen per copy)
・Reply envelope (filled in reply address and stamp attached)

When billed online

If you have a My Number Card, a smartphone that supports My Number Card, or a credit card, you can apply for a certificate with you listed online.
To use it, you need the PIN (6 to 16 digits) of the digital certificate for signature.
In addition, 300 yen per copy + postage is required.
