Could you mail a copy of the family register tag?

When requesting a copy of the family register's tag by mail, please enclose the request form, fixed-amount exchange for the fee, ordinary exchange or cash, a reply envelope with a stamp, and a copy of your identity verification document. Please send to the billing office center.

Items to be sent

・Request form (the claimant's Address, name, Phone number, permanent domicile of the certified person, name of the first person, name of proof)

※You can also download it from the city website.
※In principle, copies of family register's tag are omitted from permanent domicile and the name of the first person. If the display is required, please indicate so on the invoice.
※In the case of a request from a corporation, please indicate the name of the corporation, representative name, and location, and seal the company seal.
※In the case of a third-party request, it is necessary to indicate how to use it (the reason why a certificate is required). In addition, please enclose "Documents that can be used to confirm how to use it." In addition, certificates will be issued only to those who need it.

・Fixed amount of money, ordinary exchange (issued at the post office) or cash (registered cash) for the fee (300 yen per copy)
・Reply envelope with a stamp attached and filled in the address.
・A document that can confirm the right of proxy, such as a power of attorney if requested by the person. However, direct relatives are not required.
・Copy of identity verification documents (My Number Card, driver's license, health insurance card, etc.) with the claimant's Address

※When you send a copy of your My Number Card, please send only the surface.
※When sending a copy of your health insurance card, please send the insurer number and the insured symbol / number so that they cannot be seen by black paint.
