Where can I get a certificate of acceptance for the family register notification?

It can be taken at the "Municipal Office where family register was notified".
When we report in Yokohama-shi, we can take in ward office which we reported. (It is not possible to take it at the city hall.)
You can also request it by mail.

(In the case of marriage, divorce, adoption, adoption separation or acknowledgement, only 1,400 yen per copy if prepared using high-quality paper (B4))

・Limited to applicant who submitted family register notification. If proxy/agent is requested, a power of attorney must be submitted.

・To verify the identity of applicant, you will need to present identification documents with a photo (driver's license, passport, my number card, etc.).
・If your identity verification document does not have a photo, such as a National Health Insurance Card, you may be asked to present another identification document or ask questions.

9:00 am to midnight on the 2nd and 4th Saturday of every month
※During lunch break hours during Reception hours, except Saturday (2nd and 4th)
※Transfer holidays on Sunday, public holidays, national holidays, national holidays, New Year's holidays December 29 to January 3 of the following year
