For requests from permanent domicile City, please send a copy of the request form, fee (fixed-amount exchange, ordinary exchange or cash), reply envelope, and a copy of your identity verification documents to the Yokohama City Mail Request Office.

Items to be sent

・Request form (can also be downloaded from the city website.)
・A copy of identification documents (driver's license, health insurance card, etc.) with Address
※When sending a copy of your health insurance card, make sure that the insured person's symbol / number and insurer number are not visible by black paint.
・"Fixed exchange rate" "normal exchange" (issued at post office) or "cash" (registered cash)
※Fee is 300 yen per copy (If you are in other municipalities in permanent domicile, please contact your local municipal office.)
・Reply envelope (filled in reply address and stamp attached)
・If requested by the claimant, a power of attorney (signed by the claimant) is required. However, if the requested person is a spouse, direct ancestry, or subordinate (child, grandchild, parent, grandparent, etc.), the attachment of a power of attorney can be omitted. In this case, please indicate on the invoice that you have been requested by yourself.

(Requests from abroad)
 Fees and postage for billing from abroad cannot be paid by bank transfer. In addition, please prepare Japanese yen for cash payment. The method of sending cash by international mail varies depending on the country of origin, so please check with the post office in your country and send it by an appropriate method such as registered mail or insurance. Please note that the change will be returned with a Japanese stamp.
 If you use International Speed Mail (EMS), prepare a special envelope. In this case, please send the International Speed Mail (EMS) envelope fee of 51 yen (including tax) in addition to the handling fee and postage.

・head of family register's name (the name of the first person in family register)
・Type of certificate to be requested (please specify that you will request an ID card)
・Address and Phone number, which can be contacted during the day.
