Items to be sent

・Request form (can also be downloaded from the city website)
・A copy of the identity verification document (driver's license, health insurance card, etc.) with Address
※When sending a copy of your health insurance card, please send the insurer number and the insured symbol / number so that they cannot be seen by black paint.
・"Fixed exchange rate" "normal exchange" (issued at post office) or "cash" (registered cash)
※Fees are 450 yen per copy
※Fees may vary depending on the municipality, so if you want to charge a municipality outside Yokohama City, please contact the municipal office of the billing destination.
・Reply envelope (fill in the reply address and attach a Japanese stamp for reply)
・In addition, if requested by the claimant, a power of attorney (signed by the claimant) is required.

(Requests from abroad)
※Fees and postage for billing from abroad cannot be paid by bank transfer. In addition, please prepare Japanese yen for cash payment. The method of sending cash by international mail varies depending on the country of origin, so please check with the post office in your country and send it by an appropriate method such as registered mail or insurance. Please note that the change will be returned with a Japanese stamp.
※If you use International Speed Mail (EMS), prepare a dedicated envelope. In this case, please send the International Speed Mail (EMS) envelope fee of 51 yen (including tax) in addition to the handling fee and postage.

Details of the billing form

・Indication of permanent domicile of family register to be charged (permanent domicile address)
・head of family register's name (the name of the first person in charge of family register)
・Type and required number of certificates to be requested (excluding certificate of all matters of family register (certified copy) and certificate of personal matters (abstract). In the case of a certificate of personal matters (abstract), the name of the person to certify)
・The claimant's name, Address, and Phone number, who can be contacted during the day, and the necessary family register
・Purpose of use (in the case of personal request, the purpose of use is not required. Fees may be exempted according to laws and regulations, so if you submit it to the pension office to receive pension, please state the purpose of use.)
・If a third party (excluding the person listed in family register and a direct relative of the person listed in family register) is the claimant, please state the reason for the claim in detail.
If the reason for the request is not clear, we may ask you to provide materials.
If it is not found that there is a justifiable reason, a certificate cannot be issued.
