How do you fill out marriage registration form?

The main fields in marriage registration form are as follows:

About applicant

・Please sign your husband and wife with your maiden name.
・Both male and females can be notified by persons 18 years or older.
 However, as of April 1, 2022, women aged 16 to under 18 may submit a notification even before they reach the age of 18. In this case, parental consent is required.
(Please sign in the other column of the notification form that you agree to marriage and sign your parents. If you cannot sign, please check with "Ward Office Family Registry Division Family Registry Section". ))

About the witness

・Please sign by two adults other than applicant.
・It is not necessary to report marriages established in a foreign country.

Married married couple

・For the new permanent domicile column and post-marriage person, please choose whether to use your husband or wife.
(The selected person will be the leader of the new family register after marriage.)
・It is not possible to select both names (marital surnames)
・If the selected person is already the leader of family register, please do not fill in the New permanent domicile area.
・The new permanent domicile can be registered in any place in Japan. (The couple cannot set different locations as their permanent address.)
・If you marry a foreign national and claim to be a foreign national, you will need to submit a separate notification of his change. (marriage registration form alone can't change him.)

Parent's Name, Relationship with Parents

・Please indicate the parent's name and the relationship with the parent. (Please write the name of the adoptive parent and the relationship with the adoptive parent in the Other column.)

permanent domicile column

・Please state your domicile before marriage and the name of head of family register.

Address column

・Please indicate the Address where you are currently registered.
・If you are going to change the Address at the same time as marriage registration form and you want to submit a notification to the new Address site, please fill out the form with the new Address area and submit a notification regarding the change in Address.
(marriage registration form alone cannot change the Address. Notification of changes in Address will be handled only by the ward office Hours. For more information, please check with "Ward Office Family Registry Division Registration Section". )
