What you bring and report destinations, etc. are as follows.

What you need to bring

1 birth registration form and birth certificate
 ・Notification forms are common throughout the country.
 ・applicant is the father or mother if he is born during marriage of parents, otherwise he will be the mother. If your parents cannot become applicant, please check with the "Ward Office Family Registry Division Family Registry Section".
 ・Those who bring the written notice may be a messenger other than applicant.
 ・The right half of birth registration form is birth certificate and you'll have to be certified by your doctor or midwife. If you cannot prove your doctor or midwife, check with the ward office Family Registry Division Family Registry Section.

About characters that can be used in the child's name

There are restrictions on the characters that can be used. For more information, please check with "Ward Office Family Registry Division Family Registry Section".
(The characters that can be used are common kanji, kanji for personal names, hiragana, katakana, etc.)

notification period

・Within 14 days from the date of birth
・In the case of birth outside the country, within 3 months
・If you are a foreign national and have acquired a foreign national, you must submit a registration of nationality reservation at the same time as birth registration form within three months from the date of birth.

Reporting destination

・It will be the "Ward Office Family Registry Division Family Registry Section" in the place of birth and the location of applicant (father or mother) or the ward where permanent domicile is located.
(It cannot be handled at the Administrative Service Corner and City Hall.)
・Even if you do not have Address or permanent domicile, you can still notify the ward office where your home address is located, such as when you are staying at your parents' home.

When delivering to the ward office Hours

・We will keep the documents at the reception desk at night and holidays at the ward office, check the contents etc. in the next Hours before processing the reception process. If you cannot formally accept it, we will contact you, so be sure to include the Phone number that can be contacted during the daytime on weekdays.
・In addition, there is a page that proves that you have done birth registration form in the Maternal and Child Health Handbook, but if you send it outside the ward office Hours, you cannot prove on the spot. Please confirm handling about proof of mother and child health handbook beforehand with "ward office Family Registry Division Family Registry Section" to be delivered.
