How do I return my number card?

If you find a lost card after reissuing your My Number Card due to loss, etc., please bring your My Number Card and notify the ward office Family Registry Division Registration Section of your ward.
In addition, we also accept voluntary return of My Number Cards. Please note that a fee (1000 yen) will be charged to obtain a My Number Card again after notification by voluntary return.
Reception hours
Weekdays: From 8:45 to 17:00
2nd 4th Saturday: From 9 to 12 o'clock (noon)

Notification of persons under 15 years of age or adult ward
If you are under 15 years of age or an adult ward, please prepare the following documents and submit a report by legal representative (parental guardian, adult ward, etc.).
- Personal My Number Card
- proxy/agent's identity verification documents
- Documents proving the right of proxy
- Under 15 years old: family register Proof of all matters
- Adult ward: Adult Guardianship Registration Certificate
