Notification and certification (family register, resident certificate, etc.)

Since August 30, 2002, the resident certificate Code Notification Form has been sent to the resident certificate Code. If you need to confirm the resident certificate code, you will need to request a resident certificate Code Notification using the resident certificate Code Confirmation Issuance Request Form.
※There is no charge.

Claimer

  1. In the case of the person or same household

You will need to present your identity verification documents.

(driver's license, passport, my number card, residence card, health insurance card, pension notebook, student ID card, employee ID card, etc.)

※If necessary, you may be asked to confirm your identity.

  1. In the case of voluntary proxy/agent

It is necessary to present a power of attorney prepared by the person and the identification document of the proxy/agent. We will send the resident certificate Code Notice to your Address (Address, resident certificate) by mail.

  1. In the case of legal representative

It is necessary to present documents (such as a certificate of entry) that confirms the authority of proxy and identification documents of legal representative. We will send the resident certificate Code Notice to your Address (Address, resident certificate) by mail.

Precautions

※Currently, only those who have resident certificate in Yokohama.
※Because it is not a certificate, we do not handle it at the administrative service corner.
※This service is not available at the Family Registry Division Registration Section window where Address (resident certificate) is located.
