Where should I report death registration/death notification form? Also, what should I bring when submitting a report?

What you bring and report destinations, etc. are as follows.

For more information, please contact the ward office Family Registry Division Family Registry Section.

What you need to bring

・death registration/death notification form 1 copy (notification form is common throughout the country.)
・One copy of death certificate or postmortem certificate (created by a doctor) The right half of death registration/death notification form is death certificate, etc., and you will have to prove it there.)
・To perform cremation, please submit fire burial permission application together. (If cremation has been completed, death registration/death notification form has already been notified.)

notification period

・Within seven days from the date on which the death was learned
・In the case of death outside the country, within three months

Reporting destination

It will be the "Ward Office Family Registry Division Family Registry Section" in the place of death and the location of applicant (relatives or cohabitants) or the ward where the deceased permanent domicile is located (either one is acceptable).
(It cannot be handled at the Administrative Service Corner and City Hall.)
