In addition to the following certificate issuance counters, you can also apply online or mail requests.

Certificate Issuance Desk

・It can be taken at the "Municipal Office of the municipal office of your permanent address".
・If you have permanent domicile in Yokohama City, you can take it at any ward office or administrative service corner in the city. (We do not handle it at the city hall.)
・You can also request it by mail. Requests by phone or facsimile are not allowed.

What to bring with you at the time of request

・To verify the identity of applicant, you will need to present identification documents with a photo (driver's license, passport, my number card, etc.).
If your identity verification document does not have a photo, such as a National Health Insurance Card, you may be asked to present another identification document or ask questions.
If proxy/agent is requested, a power of attorney must be submitted, but if the requested person is a spouse, direct ancestor or subordinate (child, grandchild, parent, grandparent, etc.) of the claimant can omit the attachment of a power of attorney. In this case, please indicate on the invoice that you have been requested by yourself.

※The facility may be closed temporarily for maintenance and inspection of equipment.
※There is no parking lot, so please refrain from coming by car.

When applying online

If you have a My Number Card, a smartphone that supports My Number Card, or a credit card, you can apply for a certificate with you listed online.
To use it, you need the PIN (6 to 16 digits) of the digital certificate for signature.
In addition, 300 yen per copy + postage is required.
