Request an ID by mail
This is the procedure for obtaining an ID card by mail. It will take about 10 days to deliver it. The ID card proves that there is no entry in the bankruptcy list and that you have not been notified of registration of guardianship.

If proxy/agent requests identification of a person in Yokohama, permanent domicile requires a power of attorney or a document certifying legal representative's qualification.

What to be mailed
1. Invoice for family register Certification (for mailing)
We can download from Yokohama-shi electronic application, report service (outside site).
Notes
- The billing address stated on the invoice should be "to Director General of the relevant ward".
- Please do not use writing instruments such as pencils that can erase characters when filling in.
- Please fill in the Phone number where you can get in touch during the day. (Please note that if you have any questions about documents or billing details without Phone number's entry, we may return them.)
[If a corporation claims as proxy/agent]
- If a corporation makes a request as proxy/agent, please fill in the name, location, and title and name of the representative.
- If the person in charge of the request is not the representative, please fill in the name and Address of the person in charge.
2. Copy of identity verification document with Address

Notes
- You will need a copy of the face with your name and a copy of the face with Address.
- When sending a copy of your health insurance card, please send the insurer number and the insured person symbol / number by black paint etc. so that they cannot be seen.
- When sending a copy of your My Number Card (Personal Number Card), please send only the front surface (with photo).
- The passport cannot be used because Address cannot confirm.
3. Documents confirming authority for proxy/agent's request (power of attorney, etc.)
・The power of attorney should be prepared by the client with reference to the sample. Sample Power of Attorney (PDF: 145KB)
・If requested by legal representative, you will need documents to confirm your adult guardianship registration.
Notes
・A power of attorney is not required if your spouse, parent, grandparent, child, or grandchild is requested by you. In this case, please indicate on the invoice that the request was made.
・If a corporation makes a request as a proxy/agent, in addition to a document confirming the authority of the representative of the corporation is required. In addition, if a person other than the representative applies as a messenger of a corporate representative, a copy of the employee ID card with the company name, a power of attorney created by the representative, or a certificate of enrollment are required. For details, please contact the Yokohama City Mail Request Office.
4. Fees
300 yen per copy
Please send it by fixed amount money order / ordinary money order (handling at Japan Post Bank / Post Office savings counter) or cash (registered cash).
Notes
- We ask that you send the same amount as the fee so that you do not get any change for fixed amount currency and ordinary exchange.
- Please send the "Designated Recipient" column of the fixed amount exchange / ordinary exchange blank.
- The validity period of fixed-rate small exchange and ordinary exchange is six months from the date of issuance. We ask that you send the remaining validity period of about one week.
5. Reply envelope
Please fill in the reply address Address (the Address described in “2. Copy of Identity Confirmation Document with Address”) and attach a stamp.
The certificate will be returned to the claimant's Address (where resident certificate is located). If you wish to have another return address, please check with the Yokohama City Mail Request Office in advance.
Shipping costs
Please be aware of the amount of the stamp when creating a reply envelope. If the fee is insufficient, it will be returned by "payment of the shortage fee recipient".

Basic charge

Optional services such as express delivery and simple registered mail
If you wish to use postal services such as express delivery or simple registered mail, a separate fee will be charged. It will be returned with the sent reply envelope, so please attach the necessary stamps. For details on the fee, please check the website of Japan Post Co., Ltd. (outside site).
Not available postal services
Certificates such as family register certifications and copies of resident certificate are documents that correspond to letters (Ministry of Internal Affairs and Communications letter guidelines (external site)). Japan Post's Yu-Pack, Yu-Mail, Yu Packet, and Click Post are not available. Please note that if you specify these services for your reply, you will be required to send a reply envelope with a stamp again.
Requests from outside Japan
If you would like to receive a reply by International Express Mail (EMS), please indicate so on the invoice. In that case, the Yokohama City Mail Request Office will prepare an envelope dedicated to International Speed Mail (EMS), so please send the fee and postage plus the special envelope fee of 51 yen. (You do not need to enclose a reply envelope.)

Please note that if the fee is insufficient, the reply envelope sent from the mailing request office will be returned with the "payment of the recipient in short supply". In addition, letter pack lights, express delivery, etc. cannot be sent without a stamp for the difference, so we will ask you to postpone the stamp.

Number of days until receipt
- For billing by mail, it takes the total number of days of processing and round-trip postal delivery.
- It usually takes about 10 days from mailing to receiving the certificate, but it may take more days depending on postal circumstances and holidays, so please request in advance.
- If you are in a hurry, please consider using express mail when sending and returning documents.
- If you apply for more than one certificate at a time, or if you have any questions about the documents or billing details and need to be confirmed, it will take a few days to create the certificate. Please note that it will take more days to return the product than usual.
