Identification Card

Overview

The ID card proves that there is no entry in the bankruptcy list and that you have not been notified of registration of guardianship.
Identification cards for those with family register ID can be requested at any ward office or administrative service corner in Yokohama. You can also request it by mail.

When requesting at the counter

The person who makes the claim and the person who can do it

  1. A person who has an permanent domicile
    A proxy is not required if the spouse, parents, grandparents, children, and grandchildren of the above persons are requested by the person. In this case, please indicate on the invoice that you have been requested.

Notes

About non-computerized identification card, we cannot hand over outside Hours ID cards that are not been computerized.

What you need

  • Identification documents for the person who came to the counter family register / Identity Confirmation at the time of notification of change of residents and
    • Driver's license, passport (passport), my number card (personal number card) or Basic Resident Register card with photo, residence card or special permanent resident certificate, etc.
    • If you do not have these documents, you will need to check multiple identification documents such as "Health Insurance Card and pension's handbooks at the counter.
  • Power of Attorney (proxy/agent

When requesting by mail

Estimated number of days

  • For billing by mail, it takes the total number of processing days and round-trip postal delivery days. It usually takes about 10 days from when you mail it to when you receive the certificate, but it may take more days depending on the circumstances of the mail and holidays, so please request it in time.
  • If you are in a hurry, please consider using express mail when sending or returning documents.
  • If you apply for multiple certificates at once, or if you have any questions about the documents or billing details and need to confirm them, it will take a few days to create the certificate. Please note that it will take longer to return than usual.

Necessary when a corporation makes a request by mail

  1. Invoice
    ・Name, location, title and name of representative

    ・If a person who is actually in charge of a request is not a representative, Address is not the representative.
    ・Address, name of person who needs it (in case of family register, name of required person and permanent domicile City of the required person

  2. Documents confirming authority (a power of attorney or a document certifying that you are a legal representative
    (1) Power of Attorney
      The client has filled out and sealed all required items is sealed by the client. (The client himself is on a pre-printed version of required items's office.
      Names or seals are also acceptable.)
    (2) Documents proving that you are a legal representative
      Documents that can confirm the registration of adult guardianship, such as an adult guardianship registration certificate (original within 3 months from issuance)
    ※If you are requesting a refund of an original "Power of Attorney" or "Documents proving that you are legal representative
      Please attach a copy.
  3. Documents confirming the representative and location of the corporation
    (1) When applying for a corporate representative
      Certificate of Representative Items (In case of family register's request must be issued within 3 months of issuance)
    (2) If a person other than the representative applies
      ・A copy of the employee ID card with the company name, a power of attorney created by the representative, or a certificate of enrollment, etc.
      ・When requesting an family register is required to provide proof of the items described by the representative (original within 3 months of issuance).
    ※If you wish to return the original copy of the “Certificate of Representative Items”, please attach a copy stating that it is not different from the original.
  4. Identity verification documents
    Please enclose one of the following:
    Copy of driver's license, health insurance card, my number card with photo (personal number card), etc.
  5. Destination confirmation documents
    Please enclose one of the following:
    ・A copy of the employee ID card with the location of the head office, branch, office (destination) of the corporation
    ・Destination Address of Destination
    ・Brochures with Destination Address
    ・Certificate of registration and entry
    ・Screen print of list of establishments posted on the website
