About my number card of child younger than one year [application after December 2, 2024]
Children under one year of age who applied after December 2, 2024 will be issued a My Number card with a photo omitted. If you submit your My Number Card application along with the birth registration form notification, the card will be issued promptly and the card will be sent to your home by mail.

How to apply at the same time as birth registration form
Please fill out the required items in the “Application Form for Issuance of Personal Number Cards and Digital Certificates” in advance and submit it to the municipal office along with the birth registration form.
※Please refer to birth registration form for details on how to submit birth registration form.
Notes
If you submit an application form with birth registration form at a municipal office other than Address, it will take at least one week for the My Number Card to be sent out. Therefore, if you are in a hurry to issue a My Number Card, please submit it to the Address municipal office.

Personal Number Card Grant Application Form and Digital Certificate Issuance Application Form

- Your child's name and 1, 2, 3, and 6 are required. Please refer to the sample on page 2 and be sure to include your father, mother or legal representative in advance.
- If you would like to issue a digital certificate for user certification, enter the PIN in 1, and if you do not wish to issue it, please fill in the check box on the right. The digital certificate for user certification is required for use as a health insurance card.
- In principle, your My Number Card will be mailed to the resident registration area by simple registered mail, etc. that does not require transfer. For those who wish to temporarily send it to another place of residence for unavoidable reasons, please enter the reason in Destination and 5 in 4.
- If you have a form that integrates birth registration form with the Personal Number Card Grant Application Form and Digital Certificate Issuance Application Form, please fill out the form and submit it to the counter. There is no need to prepare a separate application for issuing a personal number card and an application for issuing a digital certificate.
- Applications submitted at the night / holiday reception desk cannot be confirmed on the spot, so they will be treated as custody. We may check the contents at a later date, so be sure to enter the contact information you can contact in 6.
Shipment of My Number Card
After submitting the application form, your My Number Card will be sent as simple registered mail, etc. that does not require transfer.
If you cannot receive mail due to absence or other reasons and the storage period at the post office has elapsed, you will be returned to the ward office Family Registry Division. After returning, please contact your ward office Family Registry Division.
About application method after birth report
After the personal number notification arrives (*), you will be able to apply for your My Number Card, so it will take about one to one and a half months from the birth notification before you get your My Number Card. Thank you for your understanding.
If you are in a hurry to use it as a health insurance card, please consult with your insurance union.
(※) Your personal number notice will be sent approximately three weeks after your birth notification.
Application using the limited express issuance system (for those who are in a hurry)
Applications can be accepted at the Family Registry Division office in your ward office. Details of how to apply will be provided on the page on the limited express issuance of My Number Card in the future.
Normal application
This is a method of having you apply by mail or online using the "Personal Number Card Grant Application Form" enclosed with the personal number notice, and receiving your My Number Card at the ward office Family Registry Division in your ward or at the My Number Card Special Center window.
It takes a few days for the My Number Card to be issued compared to the application used by the limited express issuance system.
