• (if you were born outside of Japan) within 3 months including the date of birth

• If the 14th day falls on the closing date of the ward office, the next open day will be the last day of notification period.
• If you are born outside of Japan, you may lose your Japanese nationality if you submit your birth registration form within this period, and you may lose your Japanese nationality.

In many cases, births in Japan are given by medical institutions after birth. The right half of birth registration form is birth certificate (written by a doctor or midwife), so please fill in the left half of the birth registration form you received.

In the following cases, you will need to bring your Maternal and Child Health Handbook to the registered ward office in Hours at a later date.

• When notified at night/holiday reception counter
• When Yokohama City reports non-permanent domicile at the time of opening of the office on the 2nd and 4th Saturday

Outside of reception hours at the Family Registry Division window

During this time, applications will be accepted at night and on holidays, but since the contents cannot be confirmed on the spot, it will be treated as custody. On the next opening day, the staff in charge of family register checks the contents, and if there is no defect, it will be accepted as far back as the date of acceptance, and the date of submission of birth registration form will be the "Notification Date" stated in family register.

The characters that can be used in the child's name are kanji for common use, kanji for personal names, katakana, and hiragana.

If your child is listed on resident certificate by birth registration form, you will receive a My Number. After that, a personal number notice will be sent to Address in resident certificate in about three weeks.

In birth registration form, which requires inquiries to other municipalities or other organizations, and cannot be confirmed because the contact information is closed, it will be treated as custody.
We will make an inquiry again on the next opening day, and if confirmation is confirmed, the date of submission of birth registration form will be the "Notification Date" stated in family register.
