Father or mother

- Within 14 days including the date of birth
- (If born outside of Japan) Within 3 months including the date of birth
Notes
- If the 14th day falls on the closing date of the ward office, the next opening date will be the last day of the notification period.
- If you are born abroad, you may lose your Japanese nationality if you submit your birth registration form within this period and you do not register your nationality.
What I need
1.birth registration form
In many cases, births in Japan can be obtained from medical institutions after birth. The right half of birth registration form is birth certificate (written by a doctor or midwife), so please fill in the left half of the birth registration form you received and submit it.

- If you need birth registration form paper other than the above cases, you can distribute it at the ward office Family Registry Division Family Registry Section.

2.Maternal and Child Health Handbook
Proof of birth notification in the Maternal and Child Health Handbook.
Notes
In the following cases, you will be required to bring your Maternal and Child Health Handbook to the ward office in Hours at a later date.
- When notified at the reception desk at night or on holidays
- When Yokohama City submits a notification that is not permanent domicile at the time of opening on the second and fourth Saturday

Notes
During this time, it will be accepted at night and on holidays, but it will be treated as custody because the contents cannot be confirmed on the spot. On the next opening day, family register officials will check the contents and if there are no defects, they will be accepted back to the date of acceptance, and the date of submission of birth registration form will be the “Notification Date” stated in family register.
Remarks
Characters that can be used for the child's name
The characters that can be used in the child's name are kanji for common use, kanji for personal names, katakana, and hiragana.
Please see the Ministry of Justice website (outside site) for kanji that can be used.
About my number of child
If your child is listed on resident certificate by birth registration form, my number will be assigned. After that, a personal number notice will be sent to Address in resident certificate to inform you of your My Number in about three weeks.
Notes
In birth registration form, which requires inquiries to other municipalities or other institutions, if the contact information is closed and cannot be confirmed, it will be treated as custody.
The date of submission of the birth registration form will be the “Notification Date” stated in family register.
