About re-grant due to full column or loss in postscript column of my number card

Main reasons for re-grant

  1. When the postscript column on the front of the My Number Card is full due to relocation or name change
  2. When a person who has a My Number card moves out of the country and reissues after moving in (enter Japan)
  3. If your My Number Card is lost or burned down
  4. When my number card is damaged (including poor reading of IC chip)
  5. If you are unable to continue using your My Number Card when moving in from outside the city and have expired
  6. Foreign nationals whose My Number Card has expired (excluding special permanent residents and resident status is permanent residents and second highly professional profession)

※If you are a foreigner and the expiration date of your My Number Card is approaching, you can extend the expiration date of your My Number Card. After completing the application for a residence card at the Immigration and Resident Management Agency, please bring your My Number Card and Residence Card at the ward office. If your My Number Card has expired and has expired, a reissue procedure is required (charged).

Report of Lost and Found to police department

Your personal information is listed on your My Number Card. If you lose it outside your home, please report your lost or found at police department or "Koban" (police box) before coming to the ward office. The receipt number of the lost and found report will be issued, but it will be required when reissuing your My Number Card, so please be sure to write it.

Those who need documents to prove my number

You can display your My Number on a copy of resident certificate or a certificate of entry in resident certificate.
If you are asked to present a document certifying your My Number while reissuing your My Number Card, please request a certificate of entry in resident certificate or resident certificate with your My Number.

When a lost card is found

If you have already completed the re-issuance procedure, you will not be able to use it even if you find an old card because it has been abolished. Please bring the card found when you received the new card, as it will be collected at the counter.
If the procedure for re-grant is in the future, the suspension of use will be canceled, so please take the card you find and apply at the ward office.   1. The reissuance fee is 1000 yen. You will not be able to return it after application. (800 yen if you do not have a digital certificate)
  2. Please bring identification documents with a photo such as a driver's license. If you do not have one, please bring two identification documents such as a health insurance card.
