About re-grant due to full column of postscript column of my number card or loss

Main reasons for re-issuance

  1. When the postscript column on the front of the My Number Card is full due to relocation or name change
  2. When a person who has a My Number Card moves out of the country and reissues it after moving in (entering Japan)
  3. Loss or burns your My Number Card
  4. When My Number Card is damaged (including poor reading of IC chips)
  5. In the case of transfer from outside the city, if you cannot apply for continuous use of your My Number Card and have expired
  6. Foreigners whose My Number Card has expired (excluding special permanent residents, permanent resident status, permanent resident status, and advanced professional No. 2)

※If you are a foreigner and the expiration date of your My Number Card is near, you can extend the expiration date of your My Number Card. After completing the procedures for a residence card at the Immigration and Resident Management Agency, please bring your My Number Card and Resident Card at the ward office. If your My Number Card has expired and has expired, you will need to reissue it (charged).

If you lose your My Number Card, you can use the My Number Comprehensive Toll Free Number to stop using it. If you find any loss, please call us as soon as possible.

Report of lost and found to police department

Your personal information is included on your My Number Card, so if you lose it outside your home, please report the lost and found in police department or "Koban" (police box) before coming to the ward office. The receipt number of the lost and found report will be issued, but it will be required when reissuing your My Number Card, so please be sure to write it down.

Those who need documents to prove their My Number

You can display your My Number on a copy of resident certificate or on the resident certificate entry certificate.
If a financial institution is required to present documents to prove your My Number while reissuing your My Number Card, please request a resident certificate or resident certificate entry certificate with your My Number.

If a lost card is found

If the reissuance procedure has already been completed, it will not be possible to use it even if an old card is found because it has been abolished. Please bring the card found when you receive the new card as it will be collected at the counter.
If the procedure for re-grant is in the future, we will cancel the suspension, so please bring the card you found at the ward office. If you are under 15 years of age or wish to apply by proxy/agent, please be sure to check your belongings (PDF: 149KB) (download) for cancellation of suspension of use.

  1. The re-issuance fee is 1000 yen. It cannot be returned after application. (800 yen if you do not have a digital certificate)
  2. Please bring identification documents with a photo of your face, such as a driver's license. If you do not have one, please bring two identification documents such as a health insurance card.
