We will help you apply for a My Number Card!

At four My Number Card Special Centers in the city and the ward office in your ward, we help you apply for a My Number Card, such as taking a photo of your face. In addition, special center and some ward offices can be used only for the first time application.

What you are doing to help with your application

You can complete the procedure free of charge in a minimum of 10 minutes.

Check the application form.

If you have an application for My Number Card, please bring that application.
If you do not have an application, we will reissue the application on the spot.

I will help you fill out the application form.

We will help you fill out the application form for your My Number Card.
If you are applying online from the two-dimensional code of the application form, we will help you enter it.

I'll take a photo of my face.

Take a free photo of your face to be displayed on your My Number Card.

Please bring your My Number Card Grant Application Form.

Those who do not have an application for My Number Card

Please bring your identity verification documents to reissue the application form.

Example of identity verification documents

  • Driver's license
  • Passport (passport)
  • Basic Resident Register card (with photo)
  • Residence Card or Special Permanent Resident Certificate (with photo)
  • Love Notebook (Nursing Handbook)
  • Handbook for the Physically Disabled
  • Certificate of qualification issued by a public office with a photo of the person's face attached

(Notes)

  • In the case of identity verification documents without a photo, we ask you to present multiple points. In addition, we may ask you questions.
  • Those with a validity period are limited to those within the validity period.
  • Please note that a notification card that proves your My Number will not be used as identification documents.

Reception hours

My Number Card Special Center (We are also accepting on weekends and holidays!)

You can use it at any special center, regardless of your ward.

Yokohama-sta.-nishiguchi

It can be used only for the first time.

  • Monday, Tuesday, Friday (excluding holidays)
    • From 11:30 a.m. to 7:00 p.m.
  • Saturdays, Sundays and holidays
    • 9:00 a.m. to 4:30 p.m.

※Wednesday, Thursday and Sunday, the next day on the third Saturday, are closed.

Due to congestion, you may have to wait a long time or we may refuse use on the day. Thank you for your understanding.

About receipt of my number card

After application, we send grant letter of advice (postcard) from ward office in approximately one month. When you receive the grant letter of advice (postcard), please receive it at the My Number Card Special Center or the ward office. For details, please refer to the page on how to receive My Number Card.

You may be able to receive your My Number Card by mail.

If you have a personal identification document with a photo such as a driver's license, a notification card, or a personal number notice, you may be able to receive your My Number Card by mail. Specifically, please see page of application method when we receive my number card by mail.
