I will help you apply for a My Number Card!
At the My Number Card Special Center in four locations in the city and the ward office in your ward, we help you apply for a My Number Card, such as taking a photo of your face.

What is being done to help with application
You can complete the procedure in a minimum of 10 minutes.
I will check the application form.
If you have a My Number Card application, please bring the application form.
If you do not have an application, we will reissue the application on the spot.

If you are applying online using the two-dimensional code in your application form, we will help you enter it.

Belongings
Those who have a My Number Card application form
Please bring your My Number Card Grant Application Form.
Those who do not have a My Number Card application form
Please bring your identity verification documents to reissue the application form.
Example of identity verification documents
- Car driver's license
- Passport
- Basic Resident Register card with photo
- Residence Card or Special Permanent Resident Certificate
- Nursing notebook
- Handbook for the Physically Disabled
- A license, permit, or qualification certificate issued by a public office with a photo of the person attached

- If you do not have a photo, such as a health insurance card or pension notebook, we ask you to present multiple identification documents. In addition, I would like to ask you questions.
- Items with a fixed validity period are limited to those within the validity period.
- Please note that the notification card proving your My Number will not be an identity verification document.

About receipt of my number card
After application, we send grant letter of advice (postcard) from ward office in approximately one month. When grant letter of advice (postcard) arrives, please receive at my number card special center or ward office. For more information, please refer to the page on how to receive My Number Card.
If you receive your My Number Card by mail, the application method will be different.
In order to receive your My Number Card by mail, you need to verify your identity at the time of application, and the application window is also limited. Specifically, please see page of application method when we receive my number card by mail.
