Application method when receiving my number card by mail

By applying at the My Number Card Special Center or at the ward office of Family Registry Division Registration Section (and some business offices), you can receive your My Number Card by mail at a later date.

Conditions for application

  • The applicant's visit to the Agency is required to apply. (Applications by proxy/agent are not allowed.)
    ※If the applicant is under 15 years of age or an adult ward, be sure to accompany legal representative.
  • The My Number Card Special Center is available only to those who are applying for the first time. Re-grants such as loss or renewal will be accepted only at the ward office of your ward.
  • A personal identification number will be set on the My Number Card. At a later date, when your My Number Card is completed, you will need to submit a form with your PIN at the time of application so that the counter staff will set a PIN. Thank you for your understanding.
  • It takes about one month to one and a half months to deliver your My Number Card. If there is a possibility that Address or name may change due to moving or marriage during this period, please apply after the change.
  • In the following cases, you will be asked to receive it at the counter at a later date. Thank you for your understanding.
    • The conditions of your belongings cannot be met or lacking.
    • It is difficult to pick up in Address, resident certificate (for example, you have set up a transfer at the post office).

Only those who apply for a My Number Card for the first time. Regardless of where you live, you can apply at any special center.

For legal representative and same household, presentation is not required. If you are a different household, you will need to present your family register (for foreign nationals, you will need to present it only if you are a Japanese citizen and apply at the ward office and your domicile is located in Yokohama.

  • Application for adult wardes

It is necessary to present an adult guardianship registration certificate.

Precautions

  1. We provide a free photo shooting service at the counter (it takes more time than preparing your own face photo). If you would like to use a photo taken by yourself, please prepare in advance. Please see my number card synthesis site (outside site) for details of standard of face photograph standard.
  2. Notification cards and Basic Resident Register cards will be collected at the time of application. If you have been issued in the past and have lost it outside your home, please report the lost or found in police department or "Koban" (police box) in advance (check the lost and found number at the counter). In addition, when proof of my number is necessary before receipt of my number card, please request "copy of resident certificate with my number" (300 yen per copy) at ward office or administrative service corner.
  3. In the case of a re-grant application due to loss of My Number Card, a fee (1,000 yen) will be collected at the time of application. Please note that you will not be able to return the fee after application.

Precautions

  • Documents stating "Name and date of birth" or "Name and address" issued by public offices and similar organizations can be used as identity verification documents. The information contained in the documents is consistent with resident certificate's latest information, and those with a separate expiration date must be within the expiration date.
  • Identification documents for "furigana" cannot be used. Please be careful.

Received

Shipping date

A card is created by the Local Government Information System Organization (J-LIS) and delivered to the ward office of your ward. After delivery, the staff will set a password and send it out, but it takes about one month to one and a half months from application to delivery.
If there is any defect in the application for My Number Card, a notification will be sent to the applicant from the Local Government Information System Organization (J-LIS), which is the application destination (created source) of the card. If you receive the notice, please check the contents.

Shipping method

The My Number Card will be sent out by "Personal Limited Receipt (Special Type)" or "Simplified Registered Mail" (with the consent of the individual) that allows only the person to receive mail. Please see the post office page (external site) for details on personal limited receipt mail.
The My Number Card will be sent to the applicant's Address area by mail without forwarding. If you have difficulty receiving it in the Address area of resident certificate (such as setting up a transfer at the post office), please contact the window where you applied for the issuance status, etc. and come to pick it up.
