How to apply when receiving your My Number Card by mail

You can receive your My Number Card by mail at a later date by applying at the My Number Card Special Center or at the Family Registry Division Registration Section office of your ward office (and some business offices).

Conditions of application

  • The applicant's visit to the agency is required to apply. (I will not accept any application by proxy/agent.)
    ※If the applicant is under 15 years of age or an adult ward, be sure to accompany legal representative.
  • The My Number Card Special Center is available only to those who are applying for the first time. For re-grants such as loss or renewal, only the ward office in your ward will be accepted.
  • Please note that you will be required to submit a form with your PIN at the time of application, and the staff at the counter will set a PIN at a later date.
  • It takes about one to one and a half months to deliver your My Number Card. If there is a possibility that the Address or name may change due to moving or marriage, please apply after the change.
  • In the following cases, it will be handled at the counter at a later date. Thank you for your understanding.
    • The conditions of belongings cannot be met or there is a shortage
    • It is difficult to pick up at Address in resident certificate (for example, we have set forwarding at the post office).

Only those who apply for a My Number Card for the first time can use it. You can apply at any special center regardless of your ward.

Documents that confirm the right of representation

  • Application for those under 15 years of age

In the case of legal representative and same household, presentation is not required. In the case of another household, family register (for foreign nationals, birth certificate and its translation) is required, but only if you are a Japanese citizen and apply at the ward office and your permanent domicile is in Yokohama, you do not need to present it.

  • Application for adult wardes

It is necessary to present a certificate of adult guardianship registration.

Notes

  1. Special centers and some ward offices offer free photo shooting services (it takes longer than preparing your own photo). If you would like to use a photo taken by yourself, please prepare in advance. As for the details of standard of face photograph, please see my number card synthesis site (outside site).
  2. Notification cards and Basic Resident Register cards are collected at the time of application. If you have received it in the past and have lost it outside of your home, please notify the lost and found in police department or "Koban" (police box) in advance (check the lost and found receipt number at the counter). In addition, if you need proof of your My Number before receiving your My Number Card, please request a copy of resident certificate with My Number (300 yen per copy) at the ward office or administrative service corner.
  3. In the case of re-grant application due to loss of My Number Card, a fee (1,000 yen) will be collected at the time of application. Please note that fees cannot be returned after application.

Notes

  • Documents containing "Name and date of birth" or "Name and address" issued by public offices or similar institutions can be used as identification documents. Documents that are consistent with the latest information in resident certificate and have separate expiration dates are limited to those within the expiration date.
  • You cannot use "Furigana" identification documents. Please be careful.

Received

Shipment time

A card is created by the Local Government Information System Organization (J-LIS) and delivered to the ward office in your ward. After delivery, the staff will set a PIN and ship it, but it will take about one to one and a half months from application to delivery.
If there is any deficiency in applying for a My Number Card, a notification will be sent to the applicant from the Local Government Information System Organization (J-LIS), which is the application destination (creation source) of the card. If you receive the notification, please check the details.

Shipment method

The My Number Card will be sent out by "Personal Limited Receipt (Special Type)" or "Simple Registered Mail" (if the consent of the individual) that allows only the person to receive mail. Please see the post office page (outside site) for details on personal limited receipt mail.
The My Number Card will be sent to the applicant's Address by mail that does not require forwarding. If you find it difficult to pick up at the Address area in resident certificate (for example, you have set up a transfer at the post office), you will be asked to receive it at the counter, so please contact the window where you applied for the issuance status, etc. Please come to pick it up.
