About receipt of my number card such as person under 15 years of age and adult ward

If a child under the age of 15 or an adult ward does not come to the counter and legal representative receives it, it will be handled differently. Please refer to the page about the receipt of My Number Card by proxy/agent instead of this page.

When you receive your My Number Card, please come to the counter with a person under 15 years of age or an adult ward and their legal representative.

In the case of a custody / assistant, a proxy can be received only if the right of proxy for procedures related to My Number Card can be confirmed by the proxy act list.
It is also possible to delegate receipt from legal representative to proxy/agent in addition to the visit of legal representative. If you would like to receive it by proxy/agent, please also refer to Restore proxy/agent (link in the page).

Postcards that arrive at your home by mail after applying for a My Number Card. legal representative should write the applicant's Address and name on the answer sheet on the back and bring it to the counter.

Even if you do not have a letter of delivery, you can receive it if you have the necessary identification documents. For details, please refer to the following URL.

If you have a notification card, we will collect it in exchange for your My Number Card, so please bring it to the counter when you receive it.
If you have lost it, you will need to report the lost property at police department and fill out the lost report when you receive your My Number Card.

If you have a Basic Resident Register card, we will collect it in exchange for your My Number Card, so please bring it to the counter when you receive it.
If you have lost it, you will need to report the lost property at police department and fill out the lost report when you receive your My Number Card.

If you have not returned your original My Number Card due to the full column in the update / postscript column and re-grant by departure, please bring it with you.
Please note that if there is no return, it will be reissued due to loss and a fee will be charged.
In addition, person who cannot return original my number card cannot use my number card special center.

If you are an adult ward, please bring a certificate of entry. If you are a curator or assistant, please bring a list of proxy acts in addition to the certificate of entry.
None of them are accepted by presenting copies. The certificate will be submitted or copied at the counter.

If it is difficult for legal representative to come to the agency, you can delegate the receipt to proxy/agent.
If you wish to delegate receipt to proxy/agent, please bring the documents listed in the belongings column (excluding legal representative's identity verification documents), fill in the power of attorney on the back of the letter of delivery and two identification documents (including one identification document in Category A) of proxy/agent.

If you are under the age of 15, you do not need to present a document certifying the relationship between the applicant and the legal representative only if your domicile is received at the ward office in the city. Otherwise, please bring a certificate of all matters in family register.
※The certificate will be submitted or copied at the counter.

There is a power of attorney column on the back of the delivery notice, but by creating a separate power of attorney, you can receive it without a delivery notice.
