About numbering, notice of my number (personal number)
On October 5, 2015, a person with a resident certificate in Japan was assigned a My Number (personal number). After October 5, 2015, My Number will be assigned if you submit a birth registration form or move in from abroad to create a resident certificate in Japan for the first time.
We notified my number by "my number notification card" from October 5, 2015, but the handling of new issuance, reissue, name and address change was finished on May 24, 2020. From May 25, 2020, your My Number will be notified by the "Personal Number Notice". (We will send it by mail about 3 weeks after completing procedures such as birth registration form.)

Issuance of my number notification card has been finished, but please keep it in a safe place if you have one now.
If you have lost your My Number Card application form, you can reissue it at the ward office Family Registry Division Registration Section. Specifically, please see page about application of my number card.

If you do not receive your personal number notice
About three weeks after completing procedures such as birth registration form, you will receive a personal number notice, but if delivery is not possible, we will keep it at the ward office. If you have not received it, please bring your identification documents to Family Registry Division Registration Section, the ward office of your ward.
Loss or thefts
If you have lost or stolen your My Number Notification Card or Personal Number Notification Card, please submit a lost or stolen report to police department. If there is a risk of misuse of My Number, please consult with your ward office Family Registry Division Registration Section.
In addition, the My Number Notification Card is collected in exchange for My Number Card, but if lost or stolen, it is necessary to submit a lost report when receiving the My Number Card.
If you need documents to prove your My Number
You can display your My Number on a copy of resident certificate or on the resident certificate entry certificate.
If the My Number Notification Card or Personal Number Notification Card is lost or stolen, or if the name or address of the My Number Notification Card is incorrect, and a financial institution is required to present documents proving My Number, please request a certificate of entry in resident certificate or resident certificate with My Number.
When moving out of the country
Even if you move out of the country, you will continue to use the same My Number after entering Japan. If you have a My Number Notification Card or Personal Number Notification, please keep it in a safe place after departure.
