FAQs
You will need the reservation ID indicated on the delivery notice to make a reservation for receiving your My Number Card, so please contact the ward office Family Registry Division Registration Section.
If you have already made a reservation or a Nishi Ward citizen receives it in Nishi Ward Office, you can receive your My Number Card if you do not have a letter of advice if you meet the requirements for your identity verification documents. For more information, please see the page on receiving My Number Cards.
Those that meet all of the following requirements can be used as identification documents.
- Documents marked with "Name and Date of Birth" or "Name and Address"
- Documents that match the name of Address, the name of resident certificate and the name of Address.
- Documents with an expiration date are those within the expiration date.
Please contact the ward office Family Registry Division Registration Section for actual use.
In addition to identification documents with a face photo such as a driver's license, passport, employee ID card and student ID card, you can create one of the identity verification documents with a face photo by creating a municipal-designated form called a face photo certificate .
There are three types of photo certificates: "for minors and adult guardians", "for those who are in facilities", and "for those who are receiving home care". For downloading the style, please see the page on receiving My Number Card by proxy/agent.

※If it is difficult to come to the Agency for unavoidable reasons other than the above, please contact the ward office Family Registry Division Registration Section in advance.
※Please note that in addition to documents proving the difficult reason for the next agency, it is necessary to present identification documents with a photo of the person's face.
Application for My Number Card
Download and print the address mount and insert paper for application on My Number Card General Site (outside site), and attach it to the envelope.
We send postcard called "grant letter of advice" from ward office within approximately one month after application. When you receive the delivery notice, please make a reservation for the date and time of receipt according to the guidance on the sticker attached to the delivery notice and pick it up at the My Number Card Special Center in charge of your residence area or at the window of Family Registry Division Registration Section, ward office in your residence area.
If you do not receive a letter of delivery for more than one month from your application, please contact the ward office Family Registry Division Registration Section.

If you move to another ward in Yokohama or outside Yokohama, you will need to reapply at the new Address area. Please receive reissue of application of my number card at the time of transference notice.
If your name changes due to moving or marriage in the same ward, you will be given a new Address name in the postscript column on the front of your My Number Card.
Other procedures for My Number Card
The expiration date of My Number Card will also be extended, so please bring it to the ward office Family Registry Division Registration Section. In addition, the 4-digit password will be confirmed, so please check in advance.
Please note that the card will expire after the expiration date of the original My Number Card (original expiration date), so be sure to complete the procedure before the expiration date.
If the visa is renewed and the original expiration date is reached.
If you bring your Residence Card (which proves that your visa is being renewed) and your My Number Card to the ward office Family Registry Division Registration Section, you will extend the expiration date of your My Number Card for two months. After the new visa is issued (after receiving the new residence card), please come to the extension procedure of your My Number Card again.
You can also apply at the My Number Card Special Center (open on Saturdays, Sundays and holidays) that has jurisdiction over your residential area.
In addition, by authenticating your My Number Card with the app called "JPKI PIN Reset" on your smartphone, you can reset and initialize your My Number Card PIN at a convenience store. For more information, please refer to the following link.
