Reservation is required for receipt of my number card and renewal of digital certificate, except for some ward offices. Reservations are accepted from 8:30 30 days ago.

How to make a reservation?

Receiving My Number Card

In principle, you can receive your My Number Card.

※1 Those who cannot return the original My Number Card cannot use the My Number Card Special Center.

Reservation ID

Please check your reservation ID and make a reservation using the reservation method. The reservation ID is listed on the upper left of the back of the delivery notice (postcard) received from the ward office. Please note that if the text is written by handwriting or red stamp at the beginning, that character is also included in the reservation ID.
If you do not know your reservation ID, such as when you lost it, please contact your ward office Family Registry Division Registration Section.

※From April 1, 2024, reservation system has become available. If you receive a blue envelope before January 2024, you will not be able to make a reservation, so please come directly to the counter.

Precautions

If the expiration date of the digital certificate is before April 2024, you will receive a blue envelope in the ward of the direct agency system, and those after May 2024 will receive an expiration date notice in a white envelope.

Reservation ID (reservation required)

The reservation ID is indicated on the "Expiration Notice" that arrives about three months before the expiration date. Please check your reservation ID and make a reservation using the reservation method. If you do not know your reservation ID, such as when you lost it, please contact your ward office Family Registry Division Registration Section.

A blue envelope

The reservation ID is not stated in the expiration date notice. Please check the reservation method and issue a reservation ID at the time of booking.

Please send the following contents to the My Number Card dial fax (045-350-848484). If you are able to print, please use the following fax transmission form.

Please fill in multiple dates and times as much as possible, as the desired date and time of reservation may be full.
By registering your e-mail address, you can receive the e-mail the day before the reservation has been completed and the day before the reservation.

Precautions for reservation

Since there is a reservation slot for multiple people at the same time, you may have to wait at the counter on the day. Please note that we will inform you in order.
Please come to the special center about 5 minutes before the reservation time. In addition, after the last reception time (weekdays: 7:15 pm, weekends and holidays: 4:45 pm), you will not be able to enter, so be sure to come by the final reception time.
Nishi Ward residents do not need to make a reservation in Nishi Ward Office, but if they change from the special center to pick up Nishi Ward Office, the card may be in transit. If you want to change to pick up at Nishi Ward Office before or after the special center reservation, please contact the dedicated dial.
