About my number card visit application support

For those who have difficulty going out, the staff will visit their homes and welfare facilities and help you apply for a My Number Card (advance reservation required). Cards can be picked up by mail at a later date.

Those who are eligible

Those who are applying for a My Number Card for the first time and who have difficulty going to pick up at the ward office / My Number Card Special Center

  • Those who have difficulty going out due to physical disability, illness, injury, needing nursing care, etc.
  • Those who have limited opportunities to go out due to childcare or nursing care
  • Children of infants, etc.

In companies and organizations in Yokohama City, if more than 5 people are expected, they will visit and accept applications in the same way. If you have less than 5 people, please contact us.
If you apply for your My Number Card yourself, you may be able to delegate receipt to proxy/agent. For more information, please see the page on receiving My Number Card by proxy/agent.

※If you lose your My Number Card application form, please contact your ward office Family Registry Division Registration Section.

Application Flow

1. Reservation for a visit

Reservations can be made by phone or fax.

2. Preparation of required documents

Please refer to the "What you need to prepare".

3. A staff member of Yokohama City visits on the date and time of reservation (about 30 minutes per person)

  • The staff took a photo of his face with a digital camera.
  • Helping you fill out the My Number Card Application Form
  • Confirmation of identity verification documents etc.
  • (Only for those who have one) Collection of notification cards and Basic Resident Register cards

4. Receive your My Number Card by mail

We will send it to the resident registration place by "Personal Limited Reception Mail" or "Registered Mail". (One month to one and a half months later)

What you need to prepare

Please prepare the following documents in advance. In the case of application at facilities, etc., each person planning to apply is required.

※Notification cards and Basic Resident Register cards will be collected at the time of reception.

  • Application for those under 15 years of age

For legal representative (parental parent) and same household, presentation is not required. In the case of another household, family register (for foreign nationals, birth certificate and translation) is required.

  • Application for adult wardes

It is necessary to present an adult guardianship registration certificate.

Precautions regarding identity verification documents

  • Documents stating "Name and date of birth" or "Name and address" issued by public offices and similar organizations can be used as identity verification documents. The information contained in the documents is consistent with resident certificate's latest information, and those with a separate expiration date must be within the expiration date.
  • Identification documents for "furigana" cannot be used. Please be careful.

About the reservation start date

Reservations will be accepted one month prior to the date of the visit. (Example: Reservations for visits on May 4 starts on April 4, and reservations for visits on May 5 starts on April 5.)

If you wish to make a reservation for more than 5 people at a facility, etc.

Please submit the following application documents at the time of your visit. Please prepare in advance.

About receipt of card

Shipping date

A card is created by the Local Government Information System Organization (J-LIS) and delivered to the My Number Card Special Center, which has jurisdiction over the residential area. After delivery, the staff of Yokohama City will set the password and ship it, but it takes about one month to one and a half months from visit to delivery.
If there is any defect in the application for My Number Card, a notification will be sent to the applicant from the Local Government Information System Organization (J-LIS), which is the application destination (created source) of the card. If you receive the notice, please check the contents.
 
※If proof of my number is required before receipt of my number card, please request "copy of resident certificate with my number" (300 yen per copy) at the ward office or administrative service corner.

Shipping method

The My Number Card will be sent out by "Personal Limited Reception Mail (Special Type)" or "Registered Mail" (with the consent of the individual) that allows only the person to receive mail. Please see the post office page (external site) for details on personal limited receipt mail.
In addition, My Number Card will be sent to the applicant's Address area of resident certificate by mail without forwarding.

Precautions

  • A personal identification number will be set on the My Number Card. At a later date, when your My Number Card is completed, you will need to submit a form with your PIN at the time of application so that the counter staff will set a PIN. Thank you for your understanding.
  • When using this service, you can make a reservation by a proxy, but please note that if you cannot confirm the applicant's intention, you will not be able to make a reservation.
  • At the time of the visit, if the applicant needs assistance, etc., we ask for the presence of an assistant.
  • It takes about one month to one and a half months to deliver your My Number Card. If there is a possibility that Address or name may change due to moving or marriage during this period, please apply after the change.
  • Please note that we cannot accept reservations in the following cases.
    • The conditions of your belongings cannot be met or lacking.
    • It is difficult to pick up in Address, resident certificate (for example, you have set up a transfer at the post office).
  • Since many people have made reservations, we may not be able to accept reservations at the desired date and time. We apologize for any inconvenience, but thank you for your understanding.
  • We do not require any money in My Number Card visit application support. Please be careful about fraud that took advantage of the My Number system.
