[For those who have difficulty going out] We will help you by visiting the application and receipt of your My Number Card!

For those who have difficulty going out, the staff will visit your home or welfare facilities and help you apply for a My Number Card. The card can be received by mail at a later date. Also, those who have already applied can use it. (All applicants for the first time and advance reservation system)

Notice

We are currently receiving inquiries from a very large number of people regarding reservations for visits. We accept reservations up to one month after the date of call, but we may not be able to accept reservations, so please note that we are very sorry.
You may be able to delegate your My Number Card to proxy/agent. Reservations for visits are expected to be crowded, so if you can ask for receipt, please consider proxy receipt. For more information, please refer to the page on receiving My Number Cards by proxy/agent.

Those who are eligible

Those who are applying for a My Number Card for the first time and have difficulty going to the window of the ward office / My Number Card Special Center to receive it

  • Those who have difficulty going out due to physical disability, illness, injury, nursing care, etc.
  • Those who have limited opportunities to go out due to childcare or nursing care
  • Infant children

Even in companies and organizations in Yokohama City, if more than 5 applications are expected, we will visit and accept applications in the same way. Please contact us for more than 5 applicants.
If you apply for your My Number Card yourself, you may be able to delegate your receipt to proxy/agent. For more information, please refer to the page on receiving My Number Cards by proxy/agent.

※If you have lost your My Number Card grant application form, please contact your ward office Family Registry Division Registration Section.

Flow of Procedure

1. Reservation for visits

Reservations will be accepted by phone or fax.

2. Preparation for required documents

Please refer to the "What you need to prepare" section.

3. Employees from Yokohama City visit at the reservation date and time (about 30 minutes per person)

  • The staff took a photo of the face with a digital camera, etc.
  • Helping to fill out my number card application form, etc.
  • Confirmation of identity verification documents
  • (Only those who have it) Collect notification cards and Basic Resident Register cards

4. Receipt of My Number Card by mail

It will be sent to the resident registration place by "Personal Limited Reception Mail" or "Registered Mail".

What you need to prepare

Please prepare the following documents in advance. In the case of application at facilities, etc., each person planning to apply is required.

※Notification cards and Basic Resident Register cards are collected at the time of reception.

  • Application for those under 15 years of age

In the case of legal representative (parents) and same household, presentation is not required. In other households, family register (for foreign nationals, birth certificate and translation) must be presented.

  • Application for adult wardes

It is necessary to present a certificate of adult guardianship registration.

Notes on Identity Confirmation Documents

  • Documents containing "Name and date of birth" or "Name and address" issued by public offices or similar institutions can be used as identification documents. Documents that are consistent with the latest information in resident certificate and have separate expiration dates are limited to those within the expiration date.
  • You cannot use "Furigana" identification documents. Please be careful.

About the start date of accepting reservations

Reservations will be accepted one month prior to the date of your visit. (Example: Reservations for visits on May 4 will start on April 4 and reservations for visits on May 5 will start on April 5.)

※If you have not applied and your application for My Number Card is incomplete, you will be notified to the applicant from the Local Government Information System Organization (J-LIS), which is the application destination (creating source) of the card. You. If you receive the notification, please check the details.
※If you need proof of your My Number before you receive your My Number Card, please request a copy of resident certificate with My Number (300 yen per copy) at the ward office or administrative service corner.

Shipment method

The My Number Card will be sent out by "Personal Limited Reception Mail (Special Type)" or "Registered Mail" (if the consent of the individual) that allows only the person to receive mail. Please see the post office page (outside site) for details on personal limited receipt mail.
In principle, the My Number Card will be sent to the applicant's Address area in resident certificate by mail that does not require forwarding.

Notes

  • Your personal identification number will be set on your My Number Card. At a later date, when the My Number Card is delivered to the special center, the staff at the counter will need to submit a form with the PIN at the time of application to set the PIN. Thank you for your understanding.
  • When using this service, reservations can be made by a proxy, but please note that reservations cannot be accepted if the applicant cannot be present or confirm their intention.
  • If you need assistance from the applicant at the time of the visit, we ask that an assistant be present.
  • It takes about one to one and a half months to deliver your My Number Card for those who have not yet applied. If there is a possibility that the Address or name may change due to moving or marriage, please apply after the change.
  • Please note that reservations cannot be accepted in the following cases.
    • The conditions of belongings cannot be met or there is a shortage
    • It is difficult to pick up at Address in resident certificate (for example, we have set forwarding at the post office).
  • Due to reservations from many people, we may not be able to accept reservations at the desired date and time. We apologize for any inconvenience, but we appreciate your understanding.
  • There is no request for money to support your My Number Card visit application. Please be careful about frauds that took advantage of the My Number system.
