About my face authentication My Number Card

In order to reduce the burden on those who are worried about setting and managing the PIN, there is no need to set a PIN, and the identity verification method when using the digital certificate for user certification of My Number Card is limited to face authentication or visual inspection It is my number card.
"Face authentication" is written on the face of the card, but the appearance and expiration date of the other cards are not different from the card with a PIN set.

• Use as a health insurance card

※Pre-registration is required to use it as a health insurance card. After setting facial recognition, please apply from a card reader with face recognition installed at a medical institution or pharmacy.

• Services that require you to enter your PIN

Please prepare the required documents at the time of receipt and request it at the counter. For details on the receipt procedure, please refer to Receiving My Number Card.
If you receive the proxy/agent delegated by the applicant, please indicate or select the delivery notice (postcard) that you will not set a PIN.

Applications for switching to a face authentication card can be accepted at the ward office Family Registry Division. Please prepare your My Number Card and come to the counter.

If you are going to apply for proxy/agent, you will need the documents shown in the following table.

※A power of attorney form is available on the back of the application form. Please fill in the required items and Power of Attorney on the application form and submit it to the counter in advance.

The digital certificate for signature mounted on My Number Card will be revoked upon application for changeover.

※legal representative (adult guardian) of adult ward, please bring adult guardianship registration certificate.

Procedures for switching to a card with a PIN set can be accepted at the ward office Family Registry Division. As you set (or reset) password of my number card, please come to window after preparing my number card.

If you are going to apply for proxy/agent, you will need the documents shown in the following table.

※If the proxy/agent does the procedure instead, the procedure will be based on the document inquiry method (proced later). It takes a few days to complete the procedure, so please allow yourself to complete the procedure.

1. After the procedure by proxy/agent, the ward office will mail an inquiry and response form to confirm your PIN.
2. Please fill in the signature, PIN, etc. in the response column and delegation fields of the inquiry form.
3. Please delegate to any proxy/agent to submit your My Number Card, Inquiry Form and Power of Attorney to the ward office. (proxy/agent needs to present identification documents with a face photo issued by public office.)
4. Set a new PIN at the ward office and return it to proxy/agent.
