About Limited Express Issuance of My Number Card

From December 2, 2024, cards will be issued within one week (minimum 5 days) from application for cases where it is necessary to obtain a card promptly, such as a newborn baby, loss, etc. Will be issued and sent to your home by mail will be started.

  • Transferees from abroad
  • Mid- to long-term residents who have been notified under Article 30-46 and Article 30-47 of the the Law of the Basic Resident Registers
  • Those whose personal number has been numbered or changed (excluding the above reasons)
  • Applicant for reissuance due to loss of My Number Card
  • Applicant for reissuance due to burning or damage of My Number Card
  • Those who cannot enter the changes due to the full column in the postscript column of My Number Card
  • Those who apply for a card for the first time after leaving a penal institution

Notes

  • The applicant's visit to the agency is required to apply. (You cannot apply by any proxy/agent.)

  ※If the applicant is under 15 years of age or an adult ward, be sure to accompany legal representative.

  • At the time of application, please submit a form with your PIN, and set the PIN at the local government information system organization (J-LIS), the card issuing organization.
  • Please note that it will take at least one week to send the card for those who fall under the following conditions.

  ○Those whose name kanji is treated as external characters in the system and cannot be automatically converted to the same character, or those who do not wish to convert them automatically.
  ○Those who wish to receive a facial recognition My Number Card
  ○When the maximum number of responses per day (10,000 copies per day) is exceeded nationwide

  • In addition to receiving the card by mail, you can also receive it at the counter.
  • Mail will be sent to the resident registration place in principle by simple registered mail, etc. that does not require forwarding. If you have been forwarded at the post office, please cancel the forwarding or consider receiving it at the counter.

Application period

There is an application period for limited express issuance. After the application period has elapsed, please apply for a card using the usual application method. Please confirm normal application method from my number card synthesis site (outside site).

※Please note that except for those under one year of age, those who meet the above requirements before the limited express issuance system are not eligible.
※If the 30th day falls on the closing date, the next opening date will be the application deadline.

※Congestion is expected from noon to 2:00 pm on weekdays, open on Saturday and just before the end of Reception hours, and waiting time will be longer than usual. Please come to the agency with plenty of time.

Documents that confirm the right of representation

  • Application for those under 15 years of age

In the case of legal representative and same household, presentation is not required. If you are a different household, you will need to present your family register (for foreign nationals, your birth certificate and translation), but you do not need to present it only if you are a Japanese citizen and your domicile is in Yokohama.

  • Application for adult wardes

It is necessary to present a certificate of adult guardianship registration.

Notes

  • A photo of your face will be taken at the reception desk. If you wish to use a photo taken by yourself, please bring it to the counter. Details of the face photo standard My Number Card General Site (outside site)Please take a look at it. In addition, a photo of the applicant's card under one year old will be omitted.
  • A reissue fee of 2,000 yen will be charged for applying for a reissue of My Number Card due to loss, burnout or damage, so please prepare in cash so that you do not change it.

Notes

  • Documents containing "Name and date of birth" or "Name and address" issued by public offices or similar institutions can be used as identification documents. Documents that are consistent with the latest information in resident certificate and have separate expiration dates are limited to those within the expiration date.
  • You cannot use "Furigana" identification documents. Please be careful.

About Inquiry Form and Answer Form

The inquiry form and response form for issuing My Number Card will be sent by mail from the ward office Family Registry Division in your ward.
Before you come to the window, the applicant or legal representative should request the shipment directly to the ward office Family Registry Division Registration Section.

From the reception to the delivery of the card

After the application, it will usually be sent to your home by simple registered mail, etc., which does not require transfer, from the Local Government Information System Organization (J-LIS) within about one week. If you cannot receive it at the time of delivery and the storage period at the post office has expired, it will be returned to the ward office Family Registry Division, so please contact us directly.
In addition, if there are any deficiencies in the application for My Number Card, the ward office Family Registry Division will contact you, so please check the contents and respond.
