About change of Address and name of my number card

Address or name changes, the digital certificate for signature will expire. If you would like to receive it again, please contact us.

If a same household employee applies for the issuance of a digital certificate for signature on behalf of the person, the procedure can be completed on the same day only on the same day of the move-in / move notification. Please prepare a letter of attorney and personal identification number, put it in an envelope, etc., seal it, and leave it in proxy/agent.

In the case of transfer from outside Yokohama City, your My Number Card will expire after any of the following dates, so be sure to complete the procedure by the deadline.

1. When 30 days have passed since the scheduled transfer date notified at the time of transfer in the previous Address area
2. When 14 days have passed since the date of actual transfer
3. When 90 days have passed since the date of notification of transfer

If the entry field is full, the card itself will be reissued because the new Address or name cannot be entered. Specifically, please see page about re-grant with full column of my number card or re-grant due to loss.

If you are moving out of the country, indicate in the entry column that you will move out of the country. It will be collected when you reissue your new My Number Card after returning to Japan, so please keep it in a safe place until then.
