About receipt of my number card by proxy/agent

In principle, your My Number Card will be given to the applicant, but if you cannot come to the Agency for unavoidable reasons, you can delegate receipt to proxy/agent.

About receipt by proxy/agent

The applicant of My Number Card may be delegated to proxy/agent only if it is deemed difficult to come to the Agency due to unavoidable reasons such as illness or physical disability. The main reasons for this are as follows.

  • When it is deemed difficult for the grant applicant to come to the agency due to illness, physical disability, etc.
  • When it is deemed difficult for the grant applicant to come to the agency in light of the objective situation of the work, work location, work style, etc., such as long-term (domes and overseas) business travelers, seafarers who sail for a long period of time, etc.
  • If the applicant is a junior high school student or less

Attention: Reasons such as busy work or commuting do not correspond to unavoidable reasons.

Please fill in the answer sheet column and power of attorney column on the back and bring it to the counter. If you lose it, please ask your ward office Family Registry Division Registration Section to reissue it because you cannot delegate it without a letter of delivery.

Received by proxy/agent

If you have received the proxy/agent commissioned by legal representative, please fill in the answer sheet and power of attorney on the back and bring it to the counter. In addition, since you cannot delegate without a letter of delivery, if you lose the letter of delivery, please ask the ward office Family Registry Division Registration Section of your ward to reissue it.

※Documents with "Name and Date of Birth" or "Name and Address" and whose information is consistent with the latest information of resident certificate may be used as identification documents. Documents with an expiration date are limited to those within the expiration date.
※You cannot use "Furigana" identification documents. Please be careful.
※If you are unable to use the documents not listed, or if required documents is not available, please contact Family Registry Division Registration Section in your ward office in advance.

Identification documents of the applicant

You need to bring one of the following identification documents. (Identification documents with a photo of your face are required.)

One personal identification document in Category A with a photo of your face and
1 other identification documents

1 point of identity verification document in Category B with face photo and
2 other identification documents

  • A photo certificate (for residents of facilities and home care), a long-term care insurance card, and a late-elderly Medical Certificate
  • Face photo certificate (for minors, etc.), medical care certificate and Mother and Child Handbook
  • Employee ID card (with photo), (employee health insurance) qualification confirmation and pension notebook

※If you are under one year of age at the time of application and have applied for a card after December 2, 2024, you will be issued a card with your face photo omitted, so you can receive it without a certificate with a face photo . Please prepare a total of 2 documents from categories A and B.

By attaching a photo of the applicant's face to the above form and receiving the signature or seal from the hospital or facility manager, you can make one of the identification documents (face photo certificate).

You can attach a photo of the applicant's face to the above form and receive the signature or name of the seal of the care manager (care manager) who is providing the service and the head of the business to which it belongs.

Only minors (up to 17 years old) and adult wardes can be attached to the above form and signed by legal representative to one of the identification documents (face photo certificate).

proxy/agent's identity verification documents

It is necessary to bring two identification documents, including one identification document in Category A. You will not be able to become proxy/agent unless you have a personal identification document in Category A with a photo of your face.

※Please fill in the “Response Form”, “Power of Attorney” and “PIN” on the back of the delivery notice, attach a blindfold seal to the PIN column, and leave it in proxy/agent. If the blindfold seal is not affixed, you may not be able to receive it.
※The certificate will be submitted or copied at the counter.

Documents explaining that the applicant cannot come.

If the applicant does not come to the counter and proxy/agent receives it, an explanation document is required to prove the reason. For the reasons listed in the table below, please bring the documents on the right side of the table.

  • The Long-term Care Insurance Insured Card
  • Personal number card face photo certificate created by the care manager and the head of the business to which it belongs

Original My Number Card (If you have an update or re-issue)

If you have the original My Number card in the renewal, full column in the postscript column or re-grant by re-transfer after departure, please bring it with you. If there is no return, it will be reissued due to loss and a fee will be charged.
In addition, person who does not have original my number card cannot use my number card special center.
