What should I do to get a medical checkup?

In order to receive a specific medical examination, a consultation ticket is required.

1 Select the desired medical examination institution from the list of medical examination institutions enclosed with the consultation ticket or the Yokohama City website (see the related website), etc., and contact us for reservations.
　In addition, there are some items that need to be inspected for more than 10 hours after meals (when hungry), so please check when making inquiries.

2 When you take a medical checkup, be sure to bring your "consultation ticket", "inquiry slip" and "National Health Insurance Card or My Number Card (*)".
　※ For My Number Card, you need to apply for an insurance card. Please refer to the Ministry of Health, Labour and Welfare website (outside site) (outside site) for the latest information on medical institutions that can use My Number Card insurance cards.

3 Fill out the “Inquiry Form” in advance and bring it to a medical examination organization. For question items, please circle the appropriate number of the choice.

※ If you do not know the answer or description of the question item, please bring it to a medical examination organization with blank space. (For blank areas, check with the person at the medical examination organization.)
※ If you do not have a consultation ticket or medical questionnaire, you can use the electronic application form (see related website) to issue it. If it is difficult to apply via the Internet, please contact your ward office Insurance and Pension Division National Health Insurance Section or Kenshin Dial (045-664-2606).
