In principle, the delivery of the National Health Insurance qualification transfer is based on the next agency.

However, if you have newly enrolled in social insurance due to employment, etc., and it is difficult to come to the ward office Hours for the procedure for disqualification of National Health Insurance, you can also apply for the loss by mail.

You will need to download and print registration form from the Yokohama City website. If you cannot use it, please contact Insurance and Pension Division National Health Insurance Section, ward office in your ward.
