How do I apply for a lump-sum payment for childbirth and childcare with National Health Insurance?

If you are a member of Yokohama City National Health Insurance and give birth, the application method for lump-sum childcare lump sum is as follows.

※ If you give birth to a company within six months after Retirement, you will receive a lump-sum payment for childbirth and childcare from the previously enrolled employee health insurance (only if you have been working for the company for at least one year). Depending on the employee health insurance, the amount paid may be higher than the National Health Insurance due to its own additional benefits. If you are eligible, please check with the employee health insurance you had previously subscribed to (if paid by employee health insurance, you will not be paid by the National Health Insurance).

① When giving birth at a delivery facility that handles the “direct payment system”

The amount equivalent to the lump-sum payment for childbirth and childcare (500,000 yen * 488,000 yen if not covered by the obstetric medical care compensation system) will be deducted when claiming childbirth expenses.
 As a general rule, you do not need to apply for the National Health Insurance, but if the delivery cost is less than 500,000 yen, or if you give birth at a delivery facility that is not covered by the obstetric medical care compensation system, you can apply for the difference.

※¥ 420,000 for births before March 31, 2023 (¥ 408,000 if not covered by the Obstetrics and Medical Care Compensation System)

[Application for payment of difference]

Please bring the following and apply to the ward office Insurance and Pension Division of your ward.
・Health insurance card
・Maternal and Child Health Handbook
・Deposit passbook or transfer destination can be confirmed
・Documents (receipt / statement) proving delivery costs issued by the delivery facility
・Agreement documents exchanged at the delivery facility (with a statement stating that "direct payment system will be used")

※ private seal (the name of the householder who uses vermilion) is required to transfer to an account other than the head of household.

 ② When giving birth at a delivery facility handled by the “Receiving Agency System”

After preparing a nationally designated application form at the delivery facility, apply to the ward office Insurance and Pension Division within 2 months prior to the expected delivery date.
The lump-sum childbirth allowance will be paid from the National Health Insurance to the delivery facility, so 500,000 yen of the delivery cost (488,000 yen if not covered by the obstetric medical care compensation system) will be paid at the time of discharge. It becomes unnecessary.
If the delivery cost is less than 500,000 yen, or if you give birth at a delivery facility that is not covered by the obstetric medical care compensation system, the difference will be paid.

※¥ 420,000 for births before March 31, 2023 (¥ 408,000 if not covered by the Obstetrics and Medical Care Compensation System)

・Health insurance card
・Maternal and Child Health Handbook

・Application form for lump-sum payment for childbirth and childcare (created by the delivery facility)

③ In cases where neither the “direct payment system” nor the “receiving agency system” is available (including overseas births)

[In the case of childbirth in Japan]

Please bring the following and apply to the ward office Insurance and Pension Division of your ward.
・Health insurance card
・Maternal and Child Health Handbook
・Deposit passbook or transfer destination can be confirmed
・Documents (receipt / statement) proving delivery costs issued by the delivery facility
・Agreement documents exchanged at the delivery facility (with a statement stating that "the direct payment system is not used")

※ private seal (the name of the householder who uses vermilion) is required to transfer to an account other than the head of household.

[In the case of birth overseas]

The requirement for payment is that the person who gave birth has a Address in Yokohama City and has taken out Yokohama City National Health Insurance on the day of birth.
Please bring the following and apply to the ward office Insurance and Pension Division of your ward.
•Health insurance card
•Deposit passbook or transfer destination can be confirmed
•Letter of consent form (available at the ward office.)
•Documents certifying the fact of giving birth (such as a doctor's certificate). Please attach a Japanese translation. ※2
•The original passport (passport) of the person who received treatment

※1 When transferring money to an account other than the head of household, private seal (the name of the head of household uses vermilion) is required.
※2 If you have already submitted a birth registration form and your child is already listed on resident certificate, you can omit it.
