I want to retire from the company (or because I am no longer a dependent) and get National Health Insurance.

From the date of disqualification in employee health insurance (the day after leaving the company or the day when you are no longer a dependent), you will be enrolled in Yokohama's National Health Insurance. Please submit the notification at your ward office Insurance and Pension Division National Health Insurance Section within 14 days.
In principle, the new National Health Insurance Card will be issued by mail to confirm that you live in Address, but you may be able to issue it at the counter, so please contact the ward office Insurance and Pension Division National Health Insurance Section for details. In addition, when insurance card cannot be issued window, and there is plan to be consulted immediately, as we issue "medical certificate" as substitute certificate of insurance card, please apply at window of ward office .
Note: If you retire from the company, there is a system called “voluntary continuation” that allows you to continue your workplace employee health insurance for up to two years, allowing you to choose either of the National Health Insurance. The procedure must be done by the company or employee health insurance insurer within 20 days after Retirement.

・Identity verification documents (My number card, license, passport, etc.) of the person to be notified
・Deposits and savings passbooks, financial institution registration seals
(Not required if the fund transfer procedure has been completed.)
※ Any form can be used to confirm who, what insurance and when they lost (but issued by the company or employee health insurance insurer).
