Please apply for re-grant at your ward office, Insurance and Pension Division National Health Insurance Section.
If you can confirm your identity, you will be issued at the counter, so please bring your identity verification documents.
If you are applying for a person from proxy/agent, please bring a letter of attorney (*), identification documents of the person in charge (proxy/agent) and documents that can verify the identity of the person in charge (a copy is also acceptable). If you are with resident certificate, you will be deemed to have been delegated without a power of attorney, and we will accept the procedure.
Also, if you issue a new insurance card by mail, it will take several days for you to arrive. If you plan to be consulted immediately, you can issue a “medical certificate” as a substitute for your insurance card, so please apply at the ward office.

Name of the delegate (head of household or insured) and the delegate, Address, matters of delegation, date of delegation

If you lose your insurance card due to theft, etc., please send it to the police.
