About notification of loss of National Health Insurance qualification by mail

In principle, notifications regarding the transfer of qualifications for the National Health Insurance must be made by the next agency.
However, if you have newly enrolled in social insurance due to employment, etc., and it is difficult to come to the ward office Hours for the procedure for disqualification of National Health Insurance, we will also accept the loss procedure by mail.

※Please note that you will not be able to complete the procedure if the documents you send are incomplete.
In that case, we will return the documents you sent to you.

※We are not responsible for postal accidents, so we recommend that you send it by simple registered mail.

※In cases where multiple procedures for joining or withdrawing from the National Health Insurance must be performed at the same time due to a delay in notification.
We will not be able to accept reports with the subscription procedure. About report with participation, I would like procedure in the next agency.

※Premiums may differ from the month covered by the calculation month.
After the procedure, in the fiscal year after the date of disqualification, insurance premiums may be allocated and charged, or the premium may be refunded.

※You cannot use your Yokohama National Health Insurance card after the day you join the employee health insurance of your workplace or National Health Insurance Association. If used, we may charge the medical expenses borne by Yokohama City. For more information, please refer to the following link.
