Fund transfer is convenient to pay National Health Insurance premium.

If you wish to make a fund transfer to pay your insurance premium, please bring the passbook of the account you wish to withdraw and the registration stamp of that passbook.
※For some financial institutions, procedures can only be done with a cash card.

Necessary items for notification when you quit the workplace employee health insurance or National Health Insurance Union

  1. Certificate of Disqualification of employee health insurance
    ※Get a certificate of disqualification at the workplace where you stopped or when you issued your insurance card.
    ※If you do not have the prescribed form for the certificate of disqualification, please use the employee health insurance Qualification / Loss Certificate Form (PDF: 430KB).
  2. Identification documents (my number card, driver's license, passport, etc.)

For those who have left the company due to employment reasons (such as staffing, bankruptcy due to poor business), the amount of insurance premiums and the copayment limit such as high medical costs may be reduced. For more information, please refer to the link below.

What you need for notification when you move in from outside the city

  1. Identification documents (my number card, driver's license, passport, etc.)

Necessary items for notification when a child is born

  2. Maternal and Child Health Handbook
    ※This is not required if the processing of birth has been completed by the resident registration department.

Necessary items for notification when joining the workplace employee health insurance or the National Health Insurance Association

  1. Insurance card or employee health insurance qualification certificate

If it is difficult to visit the ward office Hours due to employment, etc., we can also submit a report by mail. For more information, please refer to the notification of disqualification of National Health Insurance Qualification by mail.
You cannot use your Yokohama National Health Insurance card after the day you join the employee health insurance of your workplace or National Health Insurance Association. If used, we may charge the medical expenses borne by Yokohama City. For more information, please refer to the following link.

After the day you move out of the city, you cannot use your Yokohama National Health Insurance card. If used, we may charge the medical expenses borne by Yokohama City. For more information, please refer to the following link.

    ※Not required if the death has been completed by the resident registration department.
