Request for survivor's welfare pension
When person insured during Employee Pension Plan (Kosei Nenkin) participation and receipt incarnation of old age Employee Pension Plan (Kosei Nenkin) died, the bereaved are provided with survivor's welfare pension and are pension of reward proportion to add to survivors' basic pension, and to be provided with basically.
It was person insured or person insured, and the bereaved of the person are provided with survivor's welfare pension when we corresponded to one of the next important matters.
- When person insured died
- When while it was person insured, we died by sickness and wound with first visit (Shoshin) day within five years from first visit (Shoshin) day
- When receipt incarnation of impairment welfare pension of the first grade or the second grade and receipt incarnation of disability pension of the first grade or the second grade before revision died
- When receipt incarnation of old age Employee Pension Plan (Kosei Nenkin) or old age Employee Pension Plan (Kosei Nenkin) which met recipient qualification period and old age before revision, receipt incarnation of aggregated old-age pension died
Premium payment requirements
When there is person insured period, in the case of above, "1." and "2.", it is necessary by the month in month when the death day belongs before last about person who died to meet premium payment requirements called period when we added up premium payment finished period and premium exemption period of National Pension Plan (Kokumin Nenkin) like survivors' basic pension being two-thirds or more during the person insured period concerned.
In addition, as for this premium payment requirements, there is exception that there should not be period when we were behind with premium a year until (the month of the moon where death day belongs to about the death after May 1, 1991 before last) in the previous month in the latest standard month before month to belong on the death day in the day before on the death day when we do not meet first-term requirements when death day is before March 31, 2026.

- Being 55 years or older about husband, parents, grandparents (but we provide until 60 years old and, about parents or grandparents, are stopped).

Order of the bereaved
(1) spouse and child, (2) parents, (2) grandchild, (3) grandparents that living was maintained by person who died with the bereaved who could receive survivor's welfare pension. But there is age limit other than the wife.
When former order received supply, as for the later order, as for qualifications to receive supply, there are none.
