All pension will not be paid if they are eligible, so be sure to make a claim to the ward office National Pension Plan Section or the pension office.

(1)Those who only have the first insured period
Billing address: Ward office National Pension Plan Section

(2)Those who have the third insured period
Billing address: pension Office in charge of the Address area

Major items required for billing

  • Basic pension number notice or pension notebook
  • certificate of family register
  • resident certificate (for all households) (Note 1)
  • A bank account in the name of the claimant

Documents required depending on the case

  • Recent Premium Receipt
  • Public pension certificate (for the person and spouse)
  • Request for confirmation of pension subscription period
  • Documents certifying the amount of income (or health insurance card) (Note 2)

※What you need depends on the requester. As a general rule, attached documents can be omitted in principle because the pension Organization of Japan started full-scale operation of information sharing using My Number since July 2019.

About the system of old-age Employee Pension Plan (Kosei Nenkin) old-age Employee Pension Plan (Kosei Nenkin)

Billing address: pension Office, which oversees the Address area or the last company he worked for.

  • Prescribed medical certificate for children with severe disabilities under the age of 20
  • student registration certificate, a child under the age of 18
  • Documents certifying the amount of income (or health insurance card) (Note 2)
  • Documents confirming receipt status of Child Raising Allowance

※Please contact each mutual aid association for unpaid pension due to the death of a recipient of the mutual aid pension.
※You may be able to register at the ward office National Pension Plan Section, so please contact the pension office or the ward office National Pension Plan Section in advance.

  • The pension certificate of the deceased
  • A bank account in the name of the claimant
  • Documents confirming death (certificate of partial family register, death certificate, etc.)
  • resident certificate (for all households) of the deceased and the claimant (for same household, one card is acceptable) (Note)
  • Documents showing the relationship between the deceased and the claimant (such as certificate of family register)

Administrative work related to payment will be carried out by the Japan pension Organization.

  • Special Disability Benefit Invoice※
  • Basic pension number notice or pension notebook
  • Medical certificates, etc. for injuries and illnesses that caused disability※
  • Medical history, employment status, etc.※
  • Notification of income status of special disability benefits ※(Note 1)
  • When receiving pension, etc. from the public pension system, documents clarifying the amount received.

Documents required depending on the case

  • Certificate of enrollment (for those who were students)
  • resident certificate (Note 2)
  • Excerpt or transcript of family register (a student or a spouse of a member of Employee Pension Plan (Kosei Nenkin))
    ※The form is provided at the ward office and pension office. In addition, documents other than the above may be required depending on the situation. As a general rule, attached documents can be omitted in principle because the pension Organization of Japan started full-scale operation of information sharing using My Number since July 2019.
