The person who intends to submit a proposal shall meet all of the following conditions:

1. Being not a person listed in Article 3, Paragraph 1 of the Yokohama City Contract Regulations (March 1964 Yokohama City Regulations No. 59) and having the qualifications stipulated by the provisions of Paragraph 2 of the same Article.
2. In the Reiwa 5/6 Yokohama City General Competitive Bidding Qualified Person List (related to goods, consignment, etc.), the event is "345 office work and business consignment" (ranking is "2nd place" or more) and detailed "B: Training" A person who has registered "business" or the event must be registered with "350 other consignments" (ranking is "2nd place" or more").
3. Must not receive nomination suspension measures based on measures summary such as Yokohama-shi nomination stoppage on any day between the date of submission of the participation intention application form and the specific date of the contract candidate.
4. Have a track record of stress check and group analysis work using paper questionnaires and WEB questionnaires (within the past three years).
5. Have a person (doctor, public health nurse, etc.) prescribed in Article 52-10, Paragraph 1, Item 3 of the Occupational Safety and Health Regulations.
6. Acquired the privacy mark granted by the Japan Information Economic and Social Promotion Association. Alternatively, the same response has been made.

Notification of participation qualification confirmation result

• We will confirm the proposal qualification of the applicant who intends to participate in this proposal, and notify the proposal qualification confirmation result by e-mail by Friday, February 2, 2024, regardless of whether or not they are qualified. If we are able to confirm that you have the proposal qualification, we will also send you a proposal-related document submission request form.
• The applicant who has been notified that the proposal qualification has not been confirmed can request a written explanation of the reason why the proposal qualification was not confirmed. In addition, please submit document to City of Yokohama, Board of Education Secretariat secretariat staff labor section welfare section by 17:00 five days after excluding city hall closing agency day from the next day when Motoichi sent notice.
• When explanation is requested by the preceding paragraph, we will reply in writing to person who requested explanation within five days except city hall closing day from the next day when Motoichi received document.
