（４） Articles of Incorporation if the establisher is a corporation
（５） Medical audit results for the last three years (Results of "On-site inspections based on the provisions of Article 25, Paragraph 1 of the Medical Law"
Report or site survey report for midwifery sites) In addition, if any issues pointed out, an improvement report
（６） When it is listed in Yokohama-shi bid qualified person list, presentation of registration number.
If not listed, municipal tax tax payment certificate.
