About infectious disease surveillance system

The Infectious Disease Surveillance System is a system that shares information on the notification of outbreaks online among related parties such as medical institutions, public health centers, and prefectures. Medical institutions can report outbreaks based on the Infectious Diseases Law to the public health center by entering this system.

Medical institution (all report) Application for account creation

In order to use this system, you must agree to the terms of use and create an account.
If you have you confirm the terms of use (PDF: 253KB) in advance and agree, please apply for the creation of an account from the Yokohama City Electronic Application and Notification System Account Application Form (outside site).
After applying, an ID and password will be sent to the e-mail address entered at the time of application.

For those who already have an account

About initialization of account, other inquiries, please utilize Yokohama-shi electronic application, report system inquiry form (outside site).

All diseases subject to reporting can be reported only for accounts dedicated to all reporting.

Please note that you cannot report the occurrence of all diseases to be reported from the fixed-point report-only account.
