About infectious disease surveillance system

The Infectious Disease Surveillance System is a system in which information on outbreak reports is shared online between medical institutions, public health centers, prefectures, etc. Medical institutions can report outbreak reports based on the Infectious Diseases Law to public health centers by inputting them into this system.

Application for creating a medical institution (total report) account

In order to use this system, it is necessary to agree to the terms of use and create an account.
If you have confirmed the terms of use (PDF: 253KB) in advance and agree to it, please apply for the creation of an account from the Yokohama City Electronic Application and Notification System Account Application Form (outside site).
After applying, an ID, password, etc. will be sent to the e-mail address entered at the time of application.

For initialization of accounts and other inquiries, please use the Yokohama City Electronic Application and Notification System Inquiry Form (outside site).

For diseases to be reported in total number, only accounts dedicated to full number reporting can be reported.

Please note that it is not possible to report the occurrence of all diseases subject to reporting from a fixed-point reporting account.
