About person in charge of packaging when sending samples by Yu-Pack
When sending samples from a medical institution to a private inspection organization using Yu-Pack of Japan Post Co., Ltd., confirm that the medical institution is properly packaged, and the person in charge of certifying the “packaging person” Must be selected.
Based on "Compliance with Packaging when sending samples, etc. using a self-governing car carrier" and "Guidelines for Operation of Packaging Managers of Samples sent by Yu-Pack", one or more person at one facility Please select person in charge, and report packaging manager setting (change) notification to city.
Selection of person in charge of packaging
When the person in charge of packaging is selected, please notify the Health and Safety Division of the City of Yokohama, Medical Care Bureau Health and Safety Department without delay.
Note: Related organizations in Yokohama are subject to notification to City of Yokohama, Medical Care Bureau.
