（１） Examination office work pertaining to designation of nursing care service providers and designated renewal applications
（２） Creation of designated letter of advice of care service company and sending office work
（３） Preparatory work such as creating a designated update list for nursing care service providers
（４） Consultation services pertaining to designation of nursing care service providers and designated renewal applications
（５） Office work associated with holding group guidance workshops for nursing care service establishments
（６） Office work associated with holding new seminars for new nursing care service establishments
（７） Other duties deemed necessary by the head of the affiliation
※Including disaster response work in the event of a large-scale disaster (basically auxiliary work, only during working hours)

（１） Appointment period
April 1, 2004-March 31, 2005

（２） Working days, hours, work location
Working days　：Sunday, Saturday, one day a week (day specified by the care business guidance section manager), four days a week (30 hours) excluding national holidays and New Year's holidays (December 29-January 3)
Working hours: From 8:45 to 17:15 (break time: in principle, from 12:00 to 13:00)
Work location: City Hall 16th Floor Health and Social Welfare Bureau Nursing Care Business Guidance Section (6-50-10 Honmachi, Naka-ku, Yokohama City)
（３） Salary
¥203,400 per month　※Term-end allowance, diligence allowance, and commuting expenses (equivalent to actual expenses) are paid separately.

Annual leave, summer vacation, etc. (as per the rules on working hours, vacations, etc. of Yokohama City Fiscal Year Appointment Staff)
