We carry out help of application of my number card in various places in ward!
We will set up a support venue for My Number Card at commercial facilities in Naka Ward, and will help you with your application, such as taking a photo of your face.

What can be done at the application support venue
You can complete the procedure for free in about 10 minutes.
①I'll take a picture of your face.
Take a photo of the face displayed on my number card free of charge. Print out on the spot and paste it on the application form.
②I'll help you fill out the application form.
I will help you fill out the My Number Card application form.
If you have a My Number Card application (*), please bring the application form.
※We send application of my number card by the following method.
- Enclosed with My Number Notification Card or Personal Number Notice
- (For those under the age of 75) Mailed to those who have not yet applied for a My Number Card from February to March 2023 and from July to September 2022.
- (For those aged 75 and over) Mailed to those who have not applied for a My Number Card from mid-February to late February 2021.
If you do not have an application form, we will distribute a blank application form at the application support venue, but you will need to enter your My Number (12 digits), so please check in advance. After receiving the application form and envelope at the application support venue, you can fill it out at home.
③I will give you an envelope for sending the application form.
We hand envelope to send application to local government information system organization (my number card issuance center of country).
The shipping cost is free, so please enclose, seal it, and post it to the post.
Targets
1.Those who have resident registration in Naka Ward
※It takes about one month for my number card to be completed. In the meantime, if you move out of the ward, the card you applied for
Is invalid, so if you are planning to move out of the ward within 2 months, please apply after moving.
2.Applicants (under 15 years old and adult wardes, together with legal representative) can come to the venue
※If the applicant cannot come, you will be given only the application form.
3.Those who have never created a My Number Card before (re-grant cannot be accepted)
4.Person who has not applied for my number card grant by smartphone, PC, mail, etc.

Thing which knows my number (notification card, personal number notice, etc.) or grant application with two-dimensional code
If you do not have either a bar (notification card, personal number notice, etc.) or a two-dimensional code grant application form, we will guide you on how to take a free photo of your face and create an application form.

About receipt of my number card which we applied for
In about one month after applying, you will receive a notice (postcard) for receiving your My Number Card from the ward office.
Please follow the postcard guidance and pick it up at the My Number Card Special Center or the ward office.
