Notification of change of household (change of head of household, separation of household, merger, etc.)

Overview

This is a procedure when there is a change in the composition of the household, such as a change in the head of the household, separation or merger of households. You can accept it at your ward office, Family Registry Division Registration Section. (We cannot report in city hall, administrative service corner.)

Targets

Person who has changed the composition of the household, such as a change in the head of household, separation or merger of households.

Person who reports and can do

  • The person himself
  • Member of thesame household
  • proxy/agent (A power of attorney is required. However, this is not required if the person and a same household member submit a notification. )

Reception desk

The ward office of your ward, Family Registry Division Registration Section (You cannot submit a notification at the city hall, administrative service corner, or ward office other than your ward.)

Each ward office Family Registry Division

From 8:45 a.m. to 5:00 p.m. Monday through Friday ※Holidays and New Year holidays are excluded.
From 9:00 am to noon on the 2nd and 4th Saturday (about ward office window opening agency of the 2nd and 4th Saturday)

Application period (submission time)

Within 14 days from the date of the change
If the 14th day falls on the closing date of the ward office, the next opening date will be the last day of the notification period.

We can download style of registration form from page of Yokohama-shi electronic application, report service (outside site).
※registration form is also available at the ward office.
