About reissuance of transference certificate (certificate equivalent to transference certificate)

  • If you lose your transference certificate (certificate equivalent to the transference certificate)
  • If you are unable to submit a transfer notification using your My Number Card within 14 days from the date of moving and within 30 days from the scheduled transfer date
  • If you lose your My Number Card after the transfer notification and you cannot submit a transfer notification (exception of transfer notification) using your My Number Card

If you are unable to submit a transfer notification to the new Address due to the above reasons, we will reissue a transfer certificate (certificate equivalent to the transfer certificate).

How to apply for reissuance

Please apply by the following method.

What I need

Please bring the following when applying at the counter. Also, when applying by mail, please send the following:

  1. Transfer certificate re-grant application (PDF: 131KB)

It is also provided at the Family Registry Division office of the ward office in Address.

  1. Identification documents of the applicant

My number card, driver's license, health insurance card, etc.

※If you apply by mail, please send a copy of your identity verification documents. If you want to send a copy of your My Number Card, please send only the front (face with your face photo, Address, name, date of birth) (the part with your My Number on the back is unnecessary) ). In addition, when you send a copy of your health insurance card, please send the insurer number and the insured person symbol / number part by black paint etc.

  1. Certificate of proxy authority (only when applied by proxy/agent)

Power of Attorney (voluntary proxy/agent), certificate of family register, Certificate of Adult Guardian Registration (legal representative), etc.

※This is not necessary if you and same household members apply for re-grant in Address.

※When applying by mail, be sure to send the original power of attorney. certificate of family register, the certificate of adult guardianship registration is also acceptable.

  1. Reply envelope (only when applying by mail)

Please fill in the reply address and attach a stamp.

※If you are applying, please fill in "Address up to now" or "Future Address" notified by the transfer notice. If you wish to send it to another Address, please enclose a document that confirms the destination.

In the case of application by proxy/agent, please include a copy of your identity verification document with the destination Address or a document confirming the destination address.
