About re-grant of transference certificate (certificate equivalent to transference certificate)

  • Lost transfer certificate (certificate equivalent to the transfer certificate)
  • When the transfer notice using My Number Card cannot be submitted within 14 days from the moving date and within 30 days from the scheduled transfer date
  • If you lose your My Number Card after the transfer notice and cannot submit a transfer notice (exception of transfer notification) using your My Number Card

If the transfer notification cannot be submitted to the new Address for the above reasons, etc., a transfer certificate (certificate equivalent to the transfer certificate) will be reissued.

How to apply for re-grant

Please apply by the following method.

What you need

When applying at the counter, please bring the following items. If you wish to apply by mail, please send the following.

  1. Transference certificate re-grant application (PDF: 131KB)

It is also available at the Family Registry Division window of the ward office in Address.

  1. Identification documents for the applicant

My Number Card, Driver's License, Health Insurance Card, etc.

※When applying by mail, please send a copy of your identity verification documents. In addition, when you send a copy of your My Number Card, please send only the front side (face where your face photo, Address, name, date of birth are listed) Not required). In addition, when you send a copy of your health insurance card, please send the insurer number and the insured person symbol / number so that it is invisible by black paint.

  1. Proof of proxy authority (only when applied by proxy/agent)

Power of Attorney (optional proxy/agent), certificate of family register, Adult Guardianship Registration Certificate (legal representative)

※It is not necessary for the person and same household members to apply for re-grant in Address.

※When applying by mail, be sure to send the original power of attorney. certificate of family register, adult guardianship registration certificate is also acceptable.

  1. Envelope for reply (only when applying by mail)

Please fill in the reply address and attach a stamp.

※If you are applying, please fill in "Address Up to now" or "Future Address" notified by moving out. If you wish to send it to other Address, please enclose a document confirming the destination.

In addition, when applying by proxy/agent, please enclose a copy of the identity verification document with the recipient Address or a document confirming the destination.
