Overview
If you have changed your name, you can write your maiden name on resident certificate on request.
※The maiden name (old man) listed in resident certificate cannot be changed voluntarily.
Targets
- If you have changed your last name, you can write your maiden name on family register in resident certificate.
- If you wish to change your maiden name (old man) listed in resident certificate, you can change it to the newly created maiden name (old man) only if your last name changes after the date stated in your maiden name (old man).
- You can delete your maiden name (old Mr.) in resident certificate upon request.
- If you want to delete resident certificate's maiden name (old man) and rewrite your maiden name (old man) again, you can only write your maiden name (old man) on resident certificate after the date of deletion.
Person who reports (applications) and can do
- The person himself
- The head of the household or the person and a same household member (If you live in the same Address but have separate households, you will be treated the same as proxy/agent.)
- proxy/agent (A power of attorney is required. However, this is not required if the person and a same household member submit a notification. )

※Closed on Sunday, national holidays, and New Year's holidays.
What I need
- A copy or abstract of family register (excluded) that connects the former Mr.resident certificate with the current Mr.
※A copy of the receipt certificate, the certificate of entry, or the family register certificate cannot be accepted. In addition, the original cannot be refunded.
- Identification documents of the person who came to the counter
- My number card (one receiving grant)
※You need to write your old Mr. on the card. The old Mr. cannot be listed on the Basic Resident Register card.
- If proxy/agent is notified, you will need documents such as a power of attorney to confirm your authority.
