Write your maiden name to resident certificate

If you have changed, you can list your maiden name (old man) on resident certificate upon request.
※The maiden name listed on resident certificate cannot be arbitrarily changed.

• If you change your last name, you can enter your maiden name (old man) listed on family registerresident certificate.
• If you wish to change your maiden name (old man) listed on resident certificate, you can change it to a newly created maiden name (old man) only if your last name is changed after the date of your maiden name.
• The maiden name (old Mr.) listed on resident certificate can be deleted upon request.
• If you delete resident certificate's maiden name (Old Mr.) and wish to list your maiden name again, you may be able to enter your maiden name on resident certificate only if you change your last name after the date of deletion.

• The person himself / herself
• Head of household or person and member of same household (If you live in the same Address, you will be treated the same as proxy/agent.)
• proxy/agent (A power of attorney is required.) However, this is not required if the person and the same household staff report. )

• A transcript or abstract of family register (excluded) whose old Mr. listed in resident certificate and the present Mr.

※A copyfamily register certificate cannot be accepted. In addition, originals cannot be refunded.

• Identification documents of the person who came to the counter
• My Number Card (one who has been granted)

※You will need to write your old Mr. on your card. You cannot list your old Mr. on the Basic Resident Register card.

• When proxy/agent is submitted, documents such as a power of attorney are required to confirm the authority of proxy.
