Write your old name (nee) to resident certificate

Overview

If you have changed your name, you can write your old name in resident certificate on request.
※The old Mr. (nee) listed in resident certificate cannot be changed arbitrarily.

Targets

  • If you have changed your name, you may write your old name in family register on resident certificate.
  • If you wish to change your old name (nee) listed in resident certificate, you can change it to the newly created old name (nee) only if there is a change in your name after the date stated in your old name (nee).
  • The old Mr. (nee) listed in resident certificate may be deleted upon request.
  • If you want to delete resident certificate's old name (nee) and rewrite your old name (nee) again, you can write your old name (nee) on resident certificate only if you change your name after the date of deletion.

Person who reports and can do

  • The person himself
  • proxy/agent (A power of attorney is required.)

Reception desk

The ward office of your ward, Family Registry Division Registration Section (You cannot submit a notification at the city hall, administrative service corner, or ward office other than your ward.)

Each ward office Family Registry Division

From 8:45 a.m. to 5:00 p.m. Monday through Friday ※Holidays and closed days are excluded.
From 9:00 am to noon on the 2nd and 4th Saturday (about ward office window opening agency of the 2nd and 4th Saturday)

What I need

  • A copy or abstract of family register (excluded) that connects the former Mr.resident certificate with the current Mr.

※A copy of the receipt certificate, the certificate of entry, or the family register certificate cannot be accepted. In addition, the original cannot be refunded.

  • applicant's identity verification documents
  • My number card (person receiving grant)

※You need to write your old Mr. on the card. The old Mr. cannot be listed on the Basic Resident Register card.

  • If proxy/agent is notified, you will need documents such as a power of attorney to confirm your authority.

Please bring the above "necessary items" to the reception desk.
