Overview

It is necessary to present identification documents at the time of notification of the change of residence in family register or when requesting various certifications.

Type of identity verification documents

In principle, it is necessary to have a photo attached.

Example of identity verification documents

  • Car driver's license
  • Passport
  • My Number Card
  • Basic Resident Register card with photo
  • Residence Card or Special Permanent Resident Certificate
  • Nursing notebook
  • Handbook for the Physically Disabled
  • A license, permit, or qualification certificate issued by a public office with a photo of the person attached.

Notes

  • If you do not have a photo such as a health insurance card, pension notebook or basic pension number notice, or if you have a certificate with a photo issued by an organization other than a public office, you may be asked to present other identification documents or ask questions.
  • Those with a validity period are limited to those within the validity period.
  • Please note that the notification card that proves your My Number will not be an identity verification document.

Eligible Notifications and Certificates

Notification

  • The following notification regarding family register
    Withdrawal of marriage registration form, divorce registration form, adoption, adoption separation, acknowledgement, appeal for rejection and appeal for rejection
  • All notifications related to notification of change of residents, such as notification of transfer and notification of transfer

Certificate

  • All certificates related to family register, such as family register Certificate (certified copy) and Personal Matters Certificate (abstract)
  • All certificates related to the Basic Resident Register, including a copy of the resident certificate, a certificate of entry on the resident certificate, and a copy of the family register tag.

※When requesting Seal Registration Certificate, you only need to present your private seal registration card (card) and you do not need to present your identity verification documents.

In the case of proxy/agent

When proxy/agent requests the following certificate or submits a transfer of residents, such as moving out or moving in, it is necessary to submit documents such as a power of attorney to confirm the authority of proxy.
In principle, proxy/agent may not make an offer to withdraw the appeal for rejection appeal for rejection.

  • The following certificate for family register
