I am severely disabled. Do you have any subsidies from medical expenses?

A

For those with severe disabilities who are enrolled in employee health insurance, there is a system to subsidize the co-payment of insurance medical treatment when they go to a medical institution due to illness or injury. (Severely disabled medical expenses subsidy project)

Target person

(1) Those who have been issued a certificate of the physically disabled for grades 1 and 2

(2) Those whose intelligence index is 35 or less at the Rehabilitation Counseling Center or children's guidance office

(3) Those who have been issued a 3rd grade certificate of the physically disabled and whose intelligence index is determined to be 36 or more and 50 or less

(4) Those who have been issued the 1st grade Mental Handicapped Handbook (from October 1, 2013)

What is required for application

・Medical Certificate grant application for severely disabled people (can be filled out at the counter)

・Handbook for the physically handicapped, notebook of love, nursing notebook, etc. (those that can confirm the degree of disability), handbook for the mentally handicapped

・Confirmation of the contents of employee health insurance (conventional health insurance card, qualification confirmation, notification of qualification information, printed on the insurance card information screen of the minor portal, etc.)
